摘 要:目的探讨扶正剔邪搜络方对博莱霉素致大鼠肺间质纤维化不同时相的防治作用及其作用机制。方法将SD大鼠随机分为假手术组、模型组、激素组、扶正剔邪搜络组,气管内插管注射博莱霉素制备肺纤维化大鼠模型,假手术组注入等体积生理盐水作为正常对照,造模24 h后给予相应药物灌胃,于第7、14、28、45天分别处死,取肺组织行HE、Masson染色,ELISA法检测转化生长因子-β1(TGF-β1)含量、RT-PCR法测定Smad3、Smad4、Smad7蛋白的表达。结果扶正剔邪搜络组大鼠第7、14天肺泡炎程度均较模型组明显减轻(P<0.05),第14、28、45天纤维化程度较模型组明显减轻(P<0.05);扶正剔邪搜络组大鼠各时相TGF-β1含量均低于模型组;扶正剔邪搜络组大鼠各时相Smad3/4蛋白表达明显减弱(P<0.05),Smad7蛋白表达明显增强(P<0.05)。结论扶正剔邪搜络方不仅可以早期预防博来霉素致大鼠肺泡炎、肺纤维化的形成,长期用药还可以延缓其病程的发展,其机制可能与Smad3、Smad4信号低表达及Smad7高表达,进而影响TGF-β1通路有关。Objective To explore the preventive and therapeutic effects of Fuzheng Tixie Souluo formula, which was formulated by TCM theory of collateral disease , on rat model with pulmonary interstitial fibrosis induced by bleomyein at different time points. Methods SD rats were divided into sham- operation group, model group, hydrocortisone group and Fuzheng Tixie Souluo formula group (Chinese medicine group ) randomly. Rats were infused bleomycin through tracheal intubation to establish pulmonary fibrosis model except those in sham-operation group which were administered intragestrically the same volume of normal saline. From 24 h after modeling, sham-operation group and model group were given saline, hydrocortisone group and Chinese medicine group were given corresponding drugs. Rats were sacrificed at 7, 14, 28,45 d respectively,with the lung tissues were collected. Pathological changes of the lungs were examined by HE and Masson staining, content of transformation growth factor-β1 (TGF-β1 ) was tested by ELISA, expression of Smad3, Smad4 and Smad7 protein in the lung tissues were evaluated by RT-PCR. Results Compared with model group,the degree of alveolitis at the 7th day, 14th day and the degree of fibrosis at the 14th day, 28th day and 45th day in Chinese medicine groupdecreased remarkably(P 〈0.05 ). TGF-131 expression in Chinese medicine group was lower than that in model group at each time points. The expressions of Smad3 and Smad4 in Chinese medicine group decreased and Smad7 increased significantly(P 〈 0.01 ). Conclusion Fuzheng Tixie Souluo formula not only can prevent the formation of alveolitis and fibrosis, but also delay the progress of pulmonary fibrosis induced by bleomycin over long period. The potential mechanisms may be related to the signal transduction of TGF-13~ , attributed to the upregulation of the expression of Smad3/Smad 4 and downregulation of the expression of Smad7.
