摘  要:目的:观察参芪地黄汤对阿霉素肾病大鼠Nephrin蛋白和mRNA表达的影响,探讨参芪地黄汤减少阿霉素肾病大鼠蛋白尿的作用机制。方法:随机分为正常组、模型组、参芪地黄汤组,造模后7、14、28、56 d检测尿蛋白,造模后28、56 d检测血清白蛋白(ALB)、血肌酐(Scr),HE染色光镜下观察肾组织,免疫组化法观察Nephrin蛋白表达,RT-PCR法检测Nephrin mRNA表达。结果:造模后7、14、28、56 d,模型组大鼠24 h尿蛋白定量均显著高于正常组(P<0.05)。造模后28、56 d,参芪地黄汤组大鼠24 h尿蛋白定量明显少于模型组(P<0.05)。造模后28、56 d,模型组大鼠肾组织Nephrin蛋白和mRNA的表达与正常组相比明显下降(P<0.05)。参芪地黄汤组Nephrin蛋白和mRNA的表达较同期模型组均有显著增加(P<0.05)。结论:参芪地黄汤可能通过保留阿霉素肾病大鼠Nephrin的表达,从而减少蛋白尿,保护肾功能,改善肾病理。Objective: To investigate the effect of Shenqi Dihuang Decoction on the expression of Nephrin and to discuss the mechanism of reducing the urine protein by Shenqi Dihuang Decoction in Adriamycin( ADR)- induced nephropathy rats. Methods: The rats were randomly divided into three groups( n = 10 each group) : normal group,model group,Shenqi Dihuang Decoction group. The rats in other groups were intravenously injected with ADR( 6. 5 mg / kg) except them in normal group. The rats in Shenqi Dihuang Decoction group were administrated with Shenqi Dihuang Decoction. 24 hour urine protein was detected on day 7,14,28 and 56. Serum albumin( ALB) and serum creatinine( Scr)were detected on day 28 and 56. The morphological changes of the kidneys were observed by light microscope on day 28 and 56. The protein expression of Nephrin was determined by immunohistochemistry on day 28 and 56. The mRNA expression of Nephrin was detected by RT- PCR on day 28 and 56. Results: Compared with normal group,24 h urine protein in model group increased significantly on day 7,14,28 and 56( P〈0. 05). 24 h urine protein in Shenqi Dihuang Decoction group was much less than that in model group on day 28 and 56( P〈0. 05). Compared with normal group,the protein and mRNA expression of Nephrin decreased in model group on day 28 and 56( P〈0. 05). The protein and mRNA expression of Nephrin in Shenqi Dihuang Decoction was more than that in model group in the same period( P〈0. 05). Conclusion: Shenqi Dihuang Decoction may decrease proteinuria,protect kidney function and ameliorate histopathology in ADR- induced rats by preserving Nephrin expression.
