手术和非手术治疗锁骨中段移位骨折的效果比较

摘　　要：目的比较手术和非手术治疗锁骨中段移位骨折的临床效果。方法将389例确诊为锁骨中段移位骨折并接受治疗的患者按骨折类型分为横行骨折、斜行骨折及粉碎性骨折;将各型骨折患者按照治疗方式分为手术组及非手术组,分析和比较各组愈合率、畸形愈合率、并发症发生率及患者满意度。结果横行骨折中两组的愈合率、畸形愈合率、并发症发生率比较差异均无统计学意义(P>0.05),手术组的患者满意度为94.59%,高于非手术组的69.57%(P<0.05)。斜行骨折中手术组的并发症发生率为1.52%,低于非手术组的15.25%(P<0.05);手术组的患者满意度为89.39%,高于非手术组的57.63%(P<0.05)。粉碎性骨折中手术组的愈合率、患者满意度分别为97.52%、88.43%,高于非手术组的48.65%、27.03%(P<0.05);手术组的畸形愈合率、并发症发生率分别为2.48%、4.13%,低于非手术组的21.62%、67.57%(P<0.05)。所有骨折中手术组的愈合率、患者满意度分别为97.77%、89.73%,高于非手术组的79.39%、55.76%(P<0.05),手术组的畸形愈合率、并发症发生率分别为1.34%、3.12%,低于非手术组的6.67%、23.03%(P<0.05)。结论手术治疗可作为锁骨中段移位骨折,尤其粉碎性骨折的首选治疗方法。Objective To compare the clinical effect of operation and non-operation in the treatment of displaced midshaft clavicle fracture. Methods 389 patients with diagnosed displaced midshaft clavicle fracture were selected and divided into transverse fracture,oblique fracture and comminuted fracture according to the type of fracture.All of them were divided into operation group and non-operation group base on the result of treatment.Healing rate,malunion rate, incidence rate of complication,satisfaction rate of patient were analyzed and compared. Results The healing rate,malunion rate,incidence rate of complication in two groups of transverse fracture was compared respectively,with no statistical difference （P〉0.05）.Satisfaction rate of patient in operation group of transverse fracture was 94.59%,higher than that in non-operation group （69.57%）（P〈0.05）.lncidence rate of complication in operation group of oblique fracture was 1.52%,lower than that in non-operation（15.25%）（P〈0.05）.Satisfaction rate of patient in operation group of oblique fracture was 89.39%,higher than that in non-operation group （57.63%）（P〈0.05）.The healing rate,satisfaction rate of patient in operation group of comminuted fracture was 97.52%,88.43% respectively,higher than that in non-operation group （48.65%,27.03%） respectively （P〈0.05）.Malunion rate,incidence rate of complication in operation group of comminuted fracture was 2.48%,4.13% respectively,lower than that in non-operation group （21.62%,67.57%） respectively （P〈0.05）. The healing rate,satisfaction rate of patient in operation group of all fracture was 97.77%,89.73% respectively,higher than that in non-operation group（79.39%,55.76%） respectively（P〈0.O5）.Malunion rate,incidence rate of complication in operation group of all fracture was 1.34%,3.12% respectively,lower than that in non-operation group（6.67%,23.03%） re- spectively （P〈0.05）.
