阿立哌唑与利培酮治疗老年期精神分裂症的效果比较

摘　　要：目的比较阿立哌唑与利培酮治疗老年期精神分裂症的临床效果和安全性。方法将2013年1～12月在本院住院的60例老年期精神分裂症患者随机分为阿立哌唑与利培酮组各30例,阿立哌唑组初始剂量为5～10 mg/d,最高剂量为25 mg/d,平均（13.85±2.18）mg/d;利培酮组初始剂量为1～2 mg/d,最高剂量为6 mg/d,平均（4.28±0.78）mg/d;疗程为8周,以阳性和阴性症状量表及精神病理症状量表（PANSS）评定疗效。结果阿立哌唑组有效率为73.3%,利培酮组有效率为76.7%,差异无统计学意义（P〉0.05）。两组治疗后第2周起PANSS总分及各因子分均较治疗前降低（P〈0.05或P〈0.01）,但两组间差异无统计学意义（P〉0.05）。两组治疗后便秘、心动过速及椎体外系反应的发生率差异有统计学意义（P〈0.05）,口干、嗜睡、头痛及静坐不能的发生率差异无统计学意义（P〉0.05）。结论阿立哌唑与利培酮治疗老年期精神分裂症均由较好的临床效果,阿立哌唑的不良反应较轻,老年人大多能耐受,有良好的耐受性和安全性,更适合老年人临床中应用。Objective To compare the clinical efficacy and safety of aripiprazole and risperidone in the treatment of aged schizophrenia. Methods 60 elderly patients diagnosed as schizophrenia in our hospital from January to December 2013 were randomly divided into aripiprazole group （30 cases） and risperidone group （30 cases）.The initial dose of aripiprazole was 5-10 mg/d, the highest dose was 25 mg/d,the average was （13.85±2.18） mg/d; The initial dose of risperidone was 1-2 rag/d, the highest dose was 6 mg/d,the average was （4.28±0.78） mg/d.The course was 8-week treatment, the positive and negative syndrome scale （PANSS） was uesd to assess the efficacy. Results The treatment efficacy of aripiprazole group was 73.3%, the risperidone group was 76.7%,there was no significant difference between the two groups （P〉0.05）.The PANSS scores started to decrease since 2-week treatment in both groups, compared with before treatment, the difference was significant （P〈0.05 or P〈0.01）,but there was no significant difference between the two groups （P〉0.05）.The incidence of constipation, tachycardia and extrapyramidal reactions between the two groups had significant difference （P〈0.05）,but the incidence of dry mouth, drowsiness, headache and akathisia had no significant difference （P〉0.05）. Conclusion Aripiprazole and risperidone has equal clinical efficacy in the treatment of elderly patients with schizophrenia.Aripiprazole has less adverse reaction,thus shows better tolerability and safety.Aripiprazole is torlerable in most elderly patiens and more application in the treatment of elderly patients.
