舍曲林对糖尿病抑郁患者用药依从性及糖代谢的影响

摘　　要：目的分析舍曲林对糖尿病抑郁患者用药依从性及糖代谢的影响,为糖尿病抑郁患者的治疗提供依据。方法将90例糖尿病抑郁患者随机分为舍曲林治疗组45例和常规对照组45例,两组患者均给予常规降血糖药物治疗和糖尿病健康教育,舍曲林治疗组加用舍曲林抗抑郁治疗。采用汉密尔顿抑郁量表(HAMD)对抑郁进行评估。分别对舍曲林治疗组和常规对照组患者在治疗前和治疗8周后测定空腹血糖(FPG)、餐后2 h血糖(2 h PG)及糖化血红蛋白(Hb A1C),并进行用药依从性评定。结果两组治疗后HAMD评分、用药依从性、FPG、2 h PG及Hb A1C与同组治疗前比较,差异均有统计学意义(P<0.01,P<0.05)。舍曲林治疗组治疗后HAMD评分及临床疗效总有效率、用药依从性和FPG、2 h PG及Hb A1C与常规对照组比较,差异均有统计学意义(P<0.05,P<0.01)。结论舍曲林可显著减轻糖尿病抑郁患者的抑郁症状,提高患者用药依从性,改善糖代谢,提高患者的生活质量。Objective To analyze the influences of sertraline on medication compliance and glucose metabolism of diabetic patients with depression and to provide the base for treating depression of diabetic patients. Methods The subjects（90 diabetic patients with depression） were randomly divided into treatment group（45 cases） and control groups（45 cases）, two groups were treated with conventional hypoglycemic medication and diabetes health education, the treatment group was treated with sertraline.Both two groups were assessed by Hamilton Depression Scale（HAMD）, fasting blood glucose（FPG）, 2 hour postprandial blood glucose（ 2 h PG） and glycated hemoglobin（ Hb A1C） before treatment and after treatment for 8 weeks were detected. The medication compliance was evaluated. Results After treatment, HAMD scores, medication compliance, FPG, 2 h PG and Hb A1 Cof two groups were significantly improved（P〈 0.01 or P 〈0.05）. There were significant differences of HAMD scores, total effective rates, medication compliance, FPG, 2 h PG and Hb A1 Clevels between treatment group and control group（P〈0.01 or P〈0.05）.Conclusion Sertraline can obviously alleviate the depressive symptoms of diabetic patients with depression, raise medication compliance, improve glucose metabolism and enhance the quality of life.
