舒利迭联合布地奈德治疗老年患者重度哮喘急性发作的临床研究

摘　　要：目的 探讨舒利迭联合布地奈德治疗老年患者重度哮喘急性发作的临床应用价值。方法 将2012年1月至2013年12月间在云南省第二人民医院就诊的96例重度哮喘急性发作的患者随机分为对照组47例及观察组49例,在给予基础治疗的同时,对照组给予舒利迭50/100μg,2次/d;观察组在对照组基础上给予布地奈德400μg雾化吸入,2次/d;1个疗程为5~7 d;用药结束后评价疗效和患者咳嗽、呼吸困难、肺部啰音和喘息的消除时间,以及患者治疗前后第1秒用力呼气容积（FEV1）、最大呼吸气体的峰流速（PEF）、FEV1占肺活量比值（FEV1/FVC）的变化情况。结果 观察组总有效率明显高于对照组（87.8%比70.2%）,差异有统计学意义（χ^2=4.47,P〈0.05）;治疗后2组患者肺功能指标（FEV1、PEF和FEV1/FVC）均明显上升（P〈0.05）,且观察组改善优于对照组（P〈0.05）;观察组临床症状消失时间较短,其中咳嗽和喘息的消失时间与对照组比较差异有统计学意义（P〈0.05）。结论 舒利迭联合布地奈德治疗老年患者重度哮喘急性发作效果明显,可显著改善患者的肺功能和临床症状,值得临床推广。Objective To investigate the efficacy of seretide combined with budesonide on acute severe asthma in the elderly patients.Methods Ninety-six patients from January 2012 to November 2013 were randomly divided into observation group with 47 cases and control group with 49 cases.The control group was given seretide 50/100 μg,3 times per day by atomization inhalation,and the observation group was given seretide 50/100 u,g,3 times per day,and budesonide400 μg,2 times per day.Two groups were treated for 5-7 d.The curative effect after treatment was evaluated,and the eliminating time of cough,dyspnea,lung sound and breathing,and the levels of FEV1,PEF and FEV1/FVC were recorded.Results The total effective rate of observation group was significantly higher than that of control group（87.8%vs70.2%,χ^2 = 4.47,P〈0.05）.The levels of FEV1,PEF and FEV1/FVC of two groups were increased significantly after treatment（P〈0.05）,and the improvement of observation group was more significant than that of the control group（P〈0.05）;The eliminating time of cough,dyspnea,lung rale,breathing in observation group were lower than those in the control group（P〈0.05）.Conclusions Seretide combined with budesonide show obvious effects on severe asthma in the elderly patients,which can significantly improve lung function and clinical symptoms.
