2型糖尿病肾病患者血清血小板反应蛋白-1的临床意义

摘　　要：目的:探讨2型糖尿病肾病与血清血小板反应蛋白-1(TSP-1)的相关性。方法:将269例受试者分为糖尿病有肾脏病变组(A组,90例)、无肾脏病变组(B组,92例)和正常对照组(C组,87例),检测所有患者血清TSP-1水平及相关临床指标,并计算胰岛素抵抗指数(HOMA-IR)、胰岛β细胞功能指数(HOMA-β)。结果:A组及B组的血清TSP-1水平均显著高于C组(P<0.05),A组的血清TSP-1水平显著高于B组(P<0.05)。相关分析示血清TSP-1水平与FBG、Hb A1C、hs-CRP呈正相关(P<0.01),与HOMA-β指数及INS呈负相关(P<0.01)。多元线性逐步回归结果显示:Hb A1C,FBG,HOMA-β及INS是血清TSP-1水平的独立影响因素(P<0.05)。结论:TSP-1在2型糖尿病肾病患者血清中表达升高,其可能参与肾脏病变病理生理过程,检测血清TSP-1水平对预测糖尿病肾病有一定的价值。Objective： To explore the relationship between serum thrombospondin- 1（ TSP- 1） and kidney disease in T2 DM patients. Methods： 269 cases were divided into T2 DM patients with kidney disease group（ group A,90 cases）,T2 DM patients without kidney disease group（ group B,92 cases） and normal controls（ group C,87cases）. The serum TSP- 1 and other clinical indicators of all the cases were detected. The indexes of insulin resistance（ HOMA- IR） and the islet beta cell function index（ HOMA- β） were analyzed by steady state model. Results： The serum TSP- 1 levels in type 2 diabetes with and without kidney disease were significantly higher than those in control group（ P〈0.05）. Furthermore,among type 2 diabetes,serum TSP- 1 level was higher in patients with kidney disease than that without（ P〈0.05）. A multivariate correlation analysis showed that serum TSP- 1 level positively correlates with FBG、Hb A1 c and hs- CRP（ P〈0.01）,while negatively with HOMA- β and INS（ P〈0.01）. Stepwise linear multiple regression revealed that Hb A 1 C 、FBG 、HOMA- β and INS（ P〈0.05） were the independent factors of serum TSP- 1 level. Conclusion： The serum TSP- 1 levels are elevated in T2 DM with kidney diseas,suggesting that it might play a role in the pathophysiology of diabetic nephropath,and it is significant in predicting the microangiopathy of these patients.
