摘 要:目的:探讨被动吸烟与女性2型糖尿病(T2DM)的关系。方法:采用多阶段整群随机抽样法,抽取南昌市西湖区和青山湖区地区35-75岁之间的3 048名常住且不吸烟女性居民,由经过培训的调查人员采用面对面的方式进行被动吸烟情况现况调查和相关研究。结果:(1)在3 048名不吸烟女性中有1 392例为被动吸烟者(被动吸烟组),1 656例为非被动吸烟者(非被动吸烟组);T2DM 282例,现患率9.3%。(2)被动吸烟组糖化血红蛋白(Hb A1c)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、反映胰岛素抵抗程度的胰岛素抵抗指数(HOMA-IR)水平及T2DM患病率均显著高于非被动吸烟组(P〈0.05),而高密度脂蛋白胆固醇(HDL-C)水平则较低(P〈0.05)。(3)被动吸烟致不吸烟女性T2DM的危险性:同时调整了年龄、体重指数、糖尿病家族史、HDL-C、TC、高血压病史等混杂因素后,被动吸烟者中T2DM的比值比(OR)及95%CI可信区间为1.551(1.162-2.071),P〈0.05。结论:南昌市禁烟现状不容乐观,女性人群被动吸烟现象严重;被动吸烟可能是不吸烟女性患T2DM的独立危险因素之一,其机制可能与胰岛素抵抗有关;被动吸烟与血脂代谢紊乱亦密切相关。Objective: To examine the association between passive smoking and prevalence of type 2 diabetes mellitus( T2DM) in the non-smoking female. Methods: Representative samples were selected with multi-randomized cluster sampling among urban inhabitants aged form 35 to 75 years old in Nanchang Xihu and Qingshanhu areas. A standardized questionnaire was designed to collect information on the demographic characteristics,passive smoking history,and other risk factors of T2 DM. Results: Among the 3 048 non-smoking women,1 392 cases( 45. 7%)were exposed to passive smoking( exposure group) and 1 656 cases were not exposed to passive smoking( nonexposure group). The prevalence of T2 DM of population in these groups was 9. 3%. Compared with non-exposure group,Hb A1 c,TC,LDL-C,HOMA-IR reflecting the degree of insulin resistance,and the prevalence of T2 DM of exposure group were significantly higher( P 〈0. 05). However HDL-C in exposure group was lower. After adjusting for age,body mass index,family history of diabetes,HDL-C,TC,past histories of hypertension,the odds ratio( OR)and 95% confidence intervals( CI) for passive smokers,the T2 DM resulting from passive smokers was 1. 551( 1. 162-2. 071),P 〈0. 05. Conclusion: Smoking situation in the public is ubiquitous in Nanchang. Passive smoking may be one of independent factors for non-smoking female patients with T2 DM,whose mechanism may be associated with increased insulin resistance. And passive smoking also plays a major role in the dyslipidemia.
