左炔诺孕酮宫内缓释系统治疗功能失调性子宫出血及子宫内膜增生过长临床疗效研究

摘　　要：目的:探讨左炔诺孕酮宫内缓释系统治疗功血的临床疗效及其对子宫内膜增生过长的影响。方法:对72例年龄在31岁至50岁之间确诊为功血并有子宫内膜增生过长的病例,放置LNG-IUS进行治疗,对比治疗期间月经量、月经周期、血红蛋白水平、子宫内膜厚度等指标,并与放置前进行对比。结果:放置LNG-IUS之后,患者的月经量显著性减少,放置1年左右后降至(9±3)m L,子宫内膜厚度明显变小,由放置LNG-IUS前(0.76±0.15)cm降至(0.39±0.08)cm,放置6个月之后,子宫内膜增生过长得到改善。同时,放置LNG-IUS后患者血红蛋白水平呈上升趋势,6个月后恢复至正常状态,这些数据间的差异具有统计学意义(P<0.05),但放置前后月经周期之间的差异并不具有显著性。结论:左炔诺孕酮宫内缓释系统能有效治疗功血,减少月经量,对子宫内膜增生过长也有疗效,值得推广。Objectives： To investigate the clinical effect of levonorgestrol- releasing intrauterine system in the treatment of dysfunctional uterine bleeding and endometrium hyperplasi. Methods： 72 cases of dysfunctional uterine bleeding with endometrium hyperplasi who were 31 years to 50 years old were treated with levonorgestrol- releasing intrauterine system. The menstrual amount,menstrual cycle,hemoglobin levels and endometrial thickness during the treatment and before were compared. Results： After the treatment of the levonorgestrol- releasing intrauterine system,the menstrual amount decreased significantly,to 9 ± 3 a year after treatment; the endometrial thickness decreased from 0. 76 ± 0. 15 to 0. 39 ± 0. 08,which was improved effectively after 6 months. At the same time,the hemoglobin levels increased during the treatment,which recovered to normal state after 6 months. All the above differences were of statistical significance（ P〈0. 05）. But the difference in the menstrual cycle was not significant.Conclusion： Levonorgestrol- releasing intrauterine system can treat dysfunctional uterine bleeding and endometrium hyperplasi effectively and reduce the menstrual amount,which is worth of popularizing.
