摘　　要：目的探讨右心室间隔部主动固定电极对患者心功能的影响。方法选取我院置入永久起搏器的患者90例,将其分为右室间隔部起搏组(RVOTS组)和右心室心尖部起搏组(RVA组),术后比较两组患者的相关参数。结果两组患者术后心电图QRS时限均较术前明显升高,差异均有统计学意义(均P<0.01);两组术后左心房内径(LAD)均较术前升高,RVA组升高较为明显(P<0.05);两组术后左心室射血分数(LVEF)均较术前降低,RVA组减低明显(P<0.05);两组术后左室舒张末期内径(LVEDd)较术前均无明显变化,差异无统计学意义(P>0.05);两组术后心功能纽约心脏病协会(NYHA)分级均较术前升高,但RVA组升高明显,差异有统计学意义(P<0.01)。RVOTS组不良心血管事件发生率明显低于RVA组(P<0.05)。结论 RVOTS起搏有利于双室电激动同步,安全可靠。Objective To explore the effect of right ventricular septal pacing with active fixation lead on cardiac function.Methods A total of 90 patients in our hospital in patients with implantation of permanent pacemaker,divide it into the right ventricular septum pacing group（group RVOTS） and right ventricular apex pacing group（group RVA）, compared two groups of related parameters after operation.Results Patients of two groups after electrocardiographic QRS duration were significantly higher than the preoperative,the difference was statistically significant（P〈0.01）;the two group LAD after operation were higher than the preoperative,RVA group increased more significantly（P〈0.05）; the two group LVEF after operation were reduced,RVA group reduced significantly（P〈0.05）;the two group LVEDD after operation than before operation showed no significant change,no significant difference（P〉0.05）;the two group of postoperative heart function grade of NYHA were increased after the operation,but the RVA group increased obviously,the difference was statistically significant（P〈0.01）.The incidence of adverse cardiovascular events in RVOTS group was significantly lower than that of RVA group（P〈0.05）.Conclusion RVOTS pacing is beneficial to the double cell electrical excited synchronous,safe and reliable.
