高血压合并冠心病患者动态血压与心电图的相关性研究

摘　　要：目的探讨高血压合并冠心病患者血压波动与ST-T改变及心律失常之间的相关性。方法选取114例高血压和(或)冠心病患者分组研究,高血压合并冠心病患者36例,高血压患者40例,冠心病患者38例;所有患者均同步监测动态血压(ABPM)和动态心电图(DCG),观察各组血压波动、ST-T改变及心律失常的情况,并进行对照分析。结果高血压合并冠心病患者血压升高时心律失常、ST-T改变发生率为38.9%、64.5%,不伴高血压的冠心病组发生率为28.6%、37.0%;表明高血压合并冠心痛患者血压与其心电图ST-T改变及心律失常具有明显相关性。结论 ABPM与DCG同步检测有助于鉴别血压变化与心电活动的关系,对指导心血管疾病的诊断和治疗具有积极的临床意义。Objective To study the correlation of the fluctuation of blood pressure with the change of ST-T,cardiac arrhythmia in hypertension patients with coronary heart disease.Methods One hundred and fourteen patients with hypertension and（or） coronary heart disease were selected.Among the 114 patients,36 patients with hypertension and coronary heart disease,40 patients with hypertension and 38 patients with coronary heart disease.Synchronous 24 h ambulatory blood pressure（ABPM） and dynamic electrocardiogram（DCG） were monitored in all of the patients to observe and compare the fluctuation of blood pressure,the change of ST-T and cardiac arrhythmia among the three groups.Results The incidence of cardiac arrhythmia and the ST-T changing in hypertension patients with coronary heart disease during the blood pressure increasing was 38.9%,64.5%,and it was 28.6%,37.0% in patients with coronary heart disease.There was significant correlation between the fluctuation of blood pressure with the change of ST-T,cardiac arrhythmia in hypertension patients with coronary heart disease.Conclusion ABPM with DCG synchronous detection helps to identify the correlation between blood pressure changes and cardiacelectricalactivity,and it therefore has positive implications for clinical diagnosis and treatment guidelines for cardiovascular disease.
