摘　　要：目的 探讨不同体重指数（BMI）的老年慢性阻塞性肺疾病（COPD）患者白细胞介素（IL）-6、肿瘤坏死因子（TNF）-α、C反应蛋白（CRP）水平的变化及与缺氧的关系.方法 选取来我院就诊的老年COPD稳定期患者47例,同期参加健康体检的老年人25例（对照组）,根据BMI将COPD组分为低体重COPD组（BMI＜21 kg/m2,23例）、正常体重COPD组（BMI≥21 kg/m2,24例）.检测3组患者血清IL-6、TNF-α、CRP水平.用电发光免疫分析法检测IL-6,酶联免疫吸附试验（ELISA）法检测TNF-α,免疫比浊法检测CRP.结果 低体重COPD组IL-6、TNF-α、CRP水平明显高于正常体重COPD组和对照组,差异有统计学意义（P＜0.01）;但正常体重COPD组与对照组CRP水平比较,差异无统计学意义（P＞0.05）.低体重COPD组患者血清IL-6、TNF-α水平与BMI呈负相关,氧分压（PO2）与BMI呈正相关.结论 老年低体重COPD组患者血清IL-6、TNF-α、CRP水平明显升高,慢性缺氧及血清IL-6、TNF-α水平升高可能与COPD患者体重渐进性下降有关.Objective To investigate relationship between hypoxia and changes on serum IL-6, TNF-α and CRP levels in elderly patients with chronic obstructive pulmonary disease（COPD） with differ- ent body mass index（BMI）. Methods Select to our hospital treatment in 47 patients with stable COPD, as well as 25 cases of elderly people to participate in healthy physical examination to the same period （control group）, According to BMI, COPD groups were divided into group with underweight patients （BMI 〈 21 kg/m2, n = 23 ） and group with normal weight patients （ BMI ≥ 21 kg/m2, n = 24 ）. Serum IL-6, TNF-α and CRP levels were detected among three groups. Detection of IL-6 by electrochemiluminescent immunoassay, detection of TNF-α by ELISA, detection of CRP by immunoturbidimetry. Results The levels of serum IL-6 ,TNF-α and CRP were significantly higher in underweight COPD patients than those in normal weight patients and in healthy controls （ P 〈 0.01 ）, but serum CRP level in normal weight patients was not significantly higher than those in healthy controls （ P 〉 0.05 ）. Serum IL-6, TNF-α levels in underweight COPD patients were negatively correlated with BMI （P 〈 0.05 ）, arterial partial pressure of oxygen （PO2） was positively correlated with BMI（P 〈 0.05）. Conclusion Serum IL-6, TNF-α and CRP levels in underweight patients with COPD were significantly high, chronic hypoxia and increased serum IL-6, TNF-α levels might be correlated with COPD patients with gradual weight loss.
