结直肠癌患者癌因性疲乏的多维度纵向研究

摘　　要：目的探讨结直肠癌患者在手术和化疗过程中经历的癌因性疲乏变化规律,为针对性干预提供参考。方法采用Piper疲乏量表中文修订版(RPFS-C),对符合入选条件的115例结直肠癌患者,分别在术前及术后每个疗程开始前对癌因性疲乏进行多维度追踪随访。结果结直肠癌患者癌因性疲乏整体及感觉、认知/情绪2个维度得分在第4次化疗前呈显著上升趋势(均P<0.05),行为/严重程度、情感2个维度疲乏得分在第3次化疗前呈显著上升趋势(均P<0.05)。情感维度疲乏得分一直处于高位。行为/严重程度维度中"疲乏引起的忧虑"条目得分最低。感觉维度中"身体虚弱"条目得分最高。认知/情绪维度中"不能清晰思考"和"无记性"条目得分较低。在接受治疗后的1周内疲乏达到最高峰。结论结直肠癌患者癌因性疲乏在整个治疗阶段不断加重,临床医护人员应在围手术期及化疗早期及时开展疲乏干预,从心理支持、饮食指导和有氧运动训练等方面缓解患者的疲乏,其中情感疲乏护理是重点。Objective To describe the changing pattern of cancer related fatigue in colorectal cancer patients from before the surgery to after complete chemotherapy,and to provide reference for nursing intervention.Methods The Piper Fatigue Scale-Chinese revision（RPFS-C）was completed by 115 colorectal cancer patients before the surgery and before each course of chemotherapy.Results Significant increases in sensory fatigue,cognitive/mood fatigue,and the total fatigue were observed before the fourth stage of chemotherapy（P0.05 for all）,and the scores of behavioral fatigue and affective fatigue significantly increased before the third stage（P0.05 for both）.The score of affective domain remained far too high.The item ＂depressed＂ scored the lowest in the behavioral/severity domain.The item ＂ weak＂ scored the highest in the sensory domain.The item ＂able to concentrate＂ and ＂able to remember＂ scored the lowest in the cognitive/mood domain.The peak fatigue appeared within one week of each stage of chemotherapy.Conclusion Along with the treatment,the cancer related fatigue becomes more and more serious in colorectal cancer patients.Nursing staff should provide effective interventions such as psychological support,dietary instruction and aerobic exercise training to relieve fatigue of patients as soon as possible.And affective fatigue is the key point.
