健脾补肾方对高血压肾病的干预作用研究

摘　　要：目的观察健脾补肾方治疗高血压肾病的临床疗效。方法将90例高血压肾病患者随机分为对照组和治疗组,对照组给予常规西药降压治疗;治疗组在对照组基础上加服中药健脾补肾方,观察2组疗效及治疗前后尿蛋白、尿微量白蛋白、24 h尿蛋白定量、血清尿素氮、肌酐及血、尿β2-微球蛋白(MG)变化。结果治疗蛋白尿总有效率治疗组88%,对照组50%,2组比较差异有统计学意义(P<0.01)。治疗微量白蛋白尿总有效率治疗组81%,对照组58%,2组比较差异有统计学意义(P<0.05)。治疗组24 h尿蛋白、尿微量白蛋白、血清肌酐、尿素氮、血β2-MG治疗后均有显著下降(P<0.01或P<0.05),对照组仅尿微量白蛋白、血清肌酐有明显下降(P<0.01或P<0.05),治疗组24 h尿蛋白、尿微量白蛋白、血肌酐改善情况明显优于对照组(P<0.01或P<0.05)。结论在常规西药降压基础上给予中药健脾补肾方治疗高血压肾病疗效更显著,可明显改善蛋白尿,保护肾功能。Objective It is to observe the clinical effect of Jianpi Bushen decoction on hypertensive nephropathy. Methods 90 patients with hypertensive nephropathy were randomly divided into control group and treatment group, the control group was treated with normal antihypertensive therapy with western medicine, and the treatment group was added with Jianpi Bushen de- coction on the basis of therapy in control group. The curative effect and the changes of urinary protein, urinary microalbumin- uria, 24 - hour urinary protein quantity, BUN, SCr and 62 - MG in blood and urine before and after treatment were observed in both groups. Results The total effective rate of urinary protein was 88% in treatment group and 50% in control group, com- pared with each other the difference was significant （ P 〈 0.01 ）. The total effective rate of urinary mieroalbuminuria was 81% in treatment group and 58% in control group, compared with each other the difference was significant （P 〈0. 05）. The levels of 24 - hour urinary protein quantity, BUN, SCr and β2 - MG in blood all decreased significantly after treatment in treatment group （ P 〈0. 05 or P 〈0. 01 ） , while only the levels of urinary mieroalbuminuria and SCr decreased obviously in control group （P 〈 0.05 or P 〈 0.01 ） , the improvements of 24 - hour urinary protein quantity and SCr in treatment group were obviously better than that in control group （P 〈 0.01 or P 〈0. 05 ）. Conclusion The therapy with Jianpi Busben decoction on the basis of normal antihypertensive therapy with western medicine has a good curative effect on hypertensive nephropathy, which can obviously improve urinary protein and protect renal function.
