摘  要:目的调查分析四川泸县地区不同宫颈病变患者人类乳头状瘤病毒(Human Papilloma Virus,HPV)感染情况,为该地区HPV感染的预防和宫颈癌筛查防治提供参考。方法采用基因芯片技术对宫颈液基细胞技术所获宫颈细胞及手术切除宫颈石蜡组织进行基因分型检测,统计HPV各基因型的感染及分布情况。结果275例宫颈病变样本中,HPV检出阳性率为53.09%(146/275),其中高危感染占84.25%(123/146),低危感染占30.14%(44/146),单一感染占88.36%(129/146),混合型占11.64%(17/146)。高危型中最常见前6种亚型依次为:HPV16、39、58、52、18和66,低危型中最常见前4种亚型依次为:HPV11、6、54和44。总感染率中表现出两个年龄分布高峰,20~29岁和50。70岁年龄段。高危型感染在各年龄段没有显著差异,低危型感染主要集中在20—29岁,显著高于其余各年龄段(P〈0.01)。结论泸县地区不同程度宫颈病变患者有较高的高、低危型HPV感染率,具有较强的区域和年龄段分布特点。本研究可为该地区HPV感染的预防和宫颈癌筛查防治及疫苗使用等提供重要的参考依据。Objective To develop a better strategy to prevent human papilloma virus (HPV) infection and control cervical cancer in Luxian, we investigated the infection situation of HPV in women with different cervical lesions. Methods Using gene chip technology, HPV subtypes were detected in cervical smear or paraffin-embedded tissues from patients with cervical lesions. Results A total of 275 samples were collected in this study. The overall HPV infection rate was 53.09% (146/275), of which 84.25% (123/146) were infected by the high-risk HPV (HR-HPV) subtypes, and 30.14% (44/146) were infected by the low-risk HPV (LR-HPV) subtypes. Of the infected cases, 88.36% (129/146) and 11.64% (17/146) were infected by a single HPV subtype and mixed subtypes, respectively. The most commonly observed HR-HPV subtypes were, in a decreasing order, HPV16, 39, 58, 52, 18 and 66. The LR-HPV were HPV 11, 6, 54 and 44. The peak ages of patients with the HPV infection were from 20 to 29 and from 50 to 70 years old. There were no significant differences in HR-HPV between all age-groups, and LR-HPV was mainly concentrated in the group aged 20 to 29, which was significantly higher than the rest of all ages (P〈0.01). Conclusion The overall HPV infection rate in women with cervical lesions who lived in Luxian was high, while different ages showed different vulnerabilities to different HPV subtypes. This study provides useful information for the development of area-localized strategy for the HPV infection prevention and cervical cancer screening and vaccine use in the region.
