摘  要:目的探讨含重组人骨形态发生蛋白2(recombinant human Bone Morphogenetic Protein-2,rh BMP-2)聚乳酸(Poly Lactic Acid,PLA)缓释微球的丝素蛋白(Silk Fibroin,SF)/壳聚糖(Chitosan,CS)/纳米羟基磷灰石(nano hydroxyapatite,n-HA)支架复合部分自体松质骨修复骨缺损的疗效。方法取成年新西兰大白兔60只,随机分4组,制备右侧中段桡骨1.5cm的骨缺损模型。A组:含rh BMP-2/PLA缓释微球的SF/CS/n-HA整合支架复合部分自体松质骨,B组:含rh BMP-2的SF/CS/n-HA支架复合部分自体松质骨,C组:单纯自体松质骨,D组:单纯SF/CS/n-HA支架。术后4、8、12周摄X片、并参照Lane-Sandhu骨缺损修复组织X线评分标准,组织学、并参照Lane-Sandhu组织学评分标准进行评分,骨密度检测评估各组骨缺损修复效果。结果术后4、8、12周X片和组织学检查的结果显示,A组与C组比较差异无统计学意义(P>0.05),优于其他各组(P<0.05)。术后12周骨密度结果示A组与C组比较差异无统计学意义(P>0.05),优于其他各组(P<0.05)。结论含rh BMP-2/PLA缓释微球的SF/CS/n-HA整合支架复合部分自体松质骨与自体松质骨修复兔桡骨缺损的疗效一致,说明可以大大减少自体松质骨的使用,有望成为一种新型、优良的复合骨组织工程支架材料。Objective To explore the effect of recombinant human bone morphogenetic protein-2/poly lactic acid sustained release microspheres of Silk fibroin/chitosan/nano hydroxyapatite scaffold on section autogenous cancellous bone to repair bone defects feasibility and its efficacy. Methods We took 60 adult rabbits prepared in the middle right side of the radial bone defect model of 1.5cm. They were randomly divided into four groups. Group A: implanted recombinant human bone morphogenetic protein-2/poly lactic acid sustained release microspheres of Silk fibroin/chitosan/ nano hydroxyapatite scaffold combine section autogenous cancellous bone. Group B: implanted recombinant human bone morphogenetic protein-2 of Silk fibroin/chitosan/ nano hydroxyapatite scaffold combine section autogenous cancellous bone. Group C: implanted autogenous cancellous bone. Group D: implanted Silk fibroin/chitosan/nano hydroxyapatite scaffold. After 4, 8 and 12 weeks of surgery, the X-ray was taken and the result was evaluated with reference to Lane-Sandhu bone defect repair tissue X-ray score. Histology was evaluated with reference to Lane-Sandhu histological scoring score, and the bone density testing assessed the effect of bone defects for each group. Results After 4, 8 and 12 weeks surgery, X-ray radiography and histological results display the following. Compared with group C, group A showed no significant difference (P〉0.05), and they were better than the other groups (P〈0.05). Through 12 weeks dual-energy X-ray absorptiometry, groups A and C showed no significant difference (P〉0.05), and they were better than the other groups (P〈0.05). Conclusion In terms of the efficacy, recombinant human bone morphogenetic protein-2/poly lactic acid sustained release microspheres of Silk fibroin/chitosan/nano hydroxyapatite scaffold combine section showed consistence with autogenous cancellous bone repairing bone defect with the use the autologous cancellous bone.
