摘  要:目的了解我院重症颅脑损伤患者肺部真菌感染和药敏测试情况,指导临床用药。方法收集我院神经外科重症颅脑损伤住院患者确诊为肺部感染的呼吸道标本1 320份,对其检出276株念珠菌进行7种常用抗真菌药的体外敏感性检测。结果在检出的276株真菌中菌种分布以白色念珠菌为主187株,占67.8%,其次为热带假丝念珠菌41株,占14.8%;对7种常用药物进行耐药性分析,克柔念珠菌对氟康唑耐药率达到91.2%,卡泊芬净治疗念珠菌感染的敏感率达到100%。结论临床上念珠菌的感染和耐药菌株在不断上升,应加强念珠菌的临床检测和耐药性分析。Objective To understand the status of pulmonary fungal infection in patients with severe traumatic brain injury in our hospital and drug-sensitivity tests, so as to instruct clinical medication. Methods We collected 1320 respiratory tract specimens from the hospitalized patients who were diagnosed with lung infection and severe traumatic brain injury in the department of neurosurgery in People's Hospital, and we analyzed the drug-sensitivity tests in vitro of the seven kinds of commonly used antifungal drugs to the 276 strains of Candida which were detected from the specimens. Results From the detected 276 strains of Candida, the majority was Candida albicans with 187 strains and accounting for 67.8%, followed by Candida tropicalis with 41 strains and accounting for 14.8%. The resistance tests of the seven kinds of commonly used antifungal drugs showed that the resistant rate of Candida krusei to fluconazole reached 91.2%, and the sensitive rate of caspofungin in the treatment of Candida infection reached 100%. Conclusion The infection and the drug resistance of Candida were raising. We should strengthen the clinical testing and drug resistance analysis of Candida.
