血液透析联合血液灌流治疗尿毒症并发症临床疗效分析

摘　　要：目的分析血液透析(HD)联合血液灌流(HP)治疗尿毒症并发症的临床疗效。方法选取2011年1月至2013年1月在赤峰市医院血液净化治疗中心维持性血液透析(MHD)患者80例(透析时间大于6个月),随机分为两组,单纯HD组和HD+HP组。单纯HD组进行常规HD4h,每周3次;HD+HP组每周2次HD治疗,行HD+HP治疗1次,两组均连续治疗12周。观察两组患者治疗前后临床症状如骨关节痛、皮肤瘙痒、高血压、食欲减退、睡眠障碍等改善情况,并检测血尿素氮(BUN)、肌酐(Scr)、iPTH、β2微球蛋白(β2-MG)、血红蛋白(Hb)指标变化。结果治疗后,两组患者临床症状均有所改善,HD+HP组临床症状明显少于HD组,其中骨关节痛、皮肤瘙痒、高血压、食欲减退、睡眠障碍等临床症状改善比较差异均有统计学意义(P<0.05)。治疗后,两组患者之间相关指标均有所改善,与治疗前比较差异均有统计学意义(P<0.05)。结论血液透析联合血液灌流能够更好的清除尿毒症毒素,明显改善患者的临床症状,预防和减少透析远期并发症的发生,值得临床推广应用。Objective To analyze the treatment effect of hemodialysis combined with hemoperfusion in treatment of uremic complications. Methods Eighty MHD patients admitted to blood purification center of Chifeng Hospital were enrolled and randomly divided into two groups（dialysis time more than six months）, pure HD group and HD ＋ HP group. Pure HD group underwent conventional HD for 4 hours, 3 times a week, and based on the weekly HD series, the HD ＋ HP group took additional HP once a week, the therapeutic course in both groups bebing consecutive 12 weeks. Tow groups were observed before and after treatment in patients with clinical symptoms such as improvement of bone and joint pain, skin itching, hypertension, loss of appetite, sleep disorders and other conditions, and tested changes of blood urea nitrogen, creatinine, iPTH, 132 microglobulin, hemoglobin. Results After treatment, the clinical symptoms of HD ＋ HP group was significantly less than that in HD group, the clinical symptoms of bone and joint pain, skin itching, hypertension, less of appetite, sleep disorders improving price difference had statistical significance. The correlation index between the two groups of patients were improved compared with that before treatment, the difference had statistical significance. Conclusion Hemodialysis combined with hemoperfusion can better removal of uremic toxins, improve clinical symptoms in patients with dialysis, prevent and reduce complications, worthy of clinical application.
