缬沙坦联合氨氯地平治疗社区高血压合并糖尿病患者疗效观察

摘　　要：目的探讨社区高血压合并糖尿病患者应用缬沙坦联合氨氯地平治疗的临床疗效。方法选取2012年2月至2014年1月我院收治的原发性高血压合并糖尿病患者135例,根据随机数字表法分为三组,实验组45例给予缬沙坦联合氨氯地平的治疗,对照A组45例给予缬沙坦药物治疗,对照B组45例给予氨氯地平药物治疗,经过3个月的治疗后,比较三组患者的血压、血尿酸、空腹血糖及空腹胰岛素等情况。结果经药物治疗后,实验组有效44例,总有效率为97.8%;对照A组有效24例,总有效率为53.3%;对照B组有效26例,总有效率为57.8%。三组相比较,实验组治疗有效率显著高于其他两组,差异均有统计学意义(P<0.05)。经过药物治疗,三组患者的收缩压与舒张压均出现下降,但实验组的下降幅度明显高于其他两组,差异均有统计学意义(P<0.05)。治疗后三组患者的空腹血糖(FBG)、空腹胰岛素(FINS)、糖化血红蛋白(Hb A1c)、尿微量蛋白(Alb)均出现下降,比较下降幅度,实验组的效果更明显(P<0.05);实验组血尿酸(UA)有明显下降,改善情况明显好于其他两组(P<0.05)。结论社区高血压合并糖尿病患者给予缬沙坦联合氨氯地平的药物治疗,能够有效降低患者的血压,其治疗效果比单独用药效果更好。Objective To investigate the clinical effect of valsartan combined with amlodipine in the treatment of patients with community hypertension complicated with diabetes mellitus. Methods One hundred and thirty-five patients with primary hypertension complicated with diabetes mellitus from February 2012 to January 2014 were divided into three groups according to the random number table, each with 45 cases. The experimental group was treated with valsartan combined with amlodipine, the control group A were treated with valsartan, and the control group B were treated with amlodipine. Then the blood pressure, blood uric acid,fasting blood glucose and fasting insulin of three groups were compared at the third month after the treatment. Results The treatment was found effective in 44 patients in the experimental group（the total effective rate was 97.8%）, 24 patients in the control group A（53.3%）, and 26 patients in the control group B（57.8%）. The effective rate of the experimental group were significantly higher than those of other groups（P〈0.05）. The systolic and diastolic blood pressure declined in all the three groups after treatment, and the decline range of the experimental group were significantly larger than other groups（P〈0.05）.The fasting blood glucose（FBG）, fasting insulin（FINS）, glycosylated hemoglobin（Hb A1c） and micro-albuminuria（Alb） were all declined, and the decline range of the experimental group were larger（P〈0.05）. The blood uric acid（UA） was also found declined significantly, and the improvement of the experimental group was the best（P〈0.05）.Conclusion Valsartan combined with amlodipine can effectively decrease the blood pressure of patients with community hypertension complicated with diabetes mellitus, and its treatment effect is better than application of single drug.
