摘 要:目的探讨关节镜下有限清理联合骨赘清除治疗膝关节骨性关节炎的临床疗效。方法选择2012年1月至2014年1月我院收治的100例骨赘增生膝关节骨性关节炎患者,随机分为观察组与对照组各50例,对照组采用关节镜下有限清理术治疗,观察组采用关节镜下有限清理联合骨赘清除术治疗,记录并对比两组患者术前、术后2周、1个月、3个月的Lysholm评分和视觉模拟评分(VAS)。结果术前两组患者Lysholm评分比较差异无统计学意义(P>0.05),术后两组患者Lysholm评分均较术前明显提高,术后2周、1个月、3个月观察组Lysholm评分明显高于对照组,差异均有统计学意义(P<0.05)。术前两组患者VAS评分比较差异无统计学意义(P>0.05),术后两组患者VAS评分均较术前明显降低,术后2周、1个月、3个月观察组VAS评分明显低于对照组,差异均有统计学意义(P<0.05)。观察组总有效率为95.8%,高于对照组的71.4%,差异有统计学意义(P<0.05)。结论关节镜下有限清理联合骨赘清除治疗膝关节骨性关节炎可以有效改善患者膝关节功能,降低疼痛等症状,患者恢复情况更佳,值得临床推广应用。Objective To explore the effect of minimally invasive debridement under arthroscope combined with osteophyte removal in the treatment of knee osteoarthritis. Methods One hundred patients of knee osteoarthritis from January 2012 to January 2014 in our hospital were selected and randomly divided into the observation group and the control group, with 50 cases in each group. The control group was treated with minimally invasive debridement under arthroscope, while the observation group was treated with minimally invasive debridement under arthroscope combined with osteophyte removal. Lysholm score and visual analogue scale(VAS) were recorded and compared between the two groups before surgery, and 2 weeks, 1 month, 3 months after surgery. Results Lysholm score showed no statistically significant difference between the two groups before treatment(P〉0.05). The score were significantly increased after treatment in the two groups, which was significantly higher in the observation group than the control group 2week, 1 month, 3 months after surgery(P〈0.05). VAS scores of two groups had no statistically significant difference before treatment(P〉0.05), and they were significantly decreased after treatment. The score was significantly lower in the observation group than the control group 2 weeks, 1 month, 3 months after surgery(P〈0.05). The total effective rate of the observation group was 95.8%, significantly higher than 71.4% of the control group(P〈0.05). Conclusion Minimally invasive debridement under arthroscope combined with osteophyte removal in the treatment of knee osteoarthritis can effectively improve the patients' knee joint functions, reduce pain and other symptoms, with better recovery,which is worth of clinical application.
