角型钛夹在下鼻甲部分切除术中的临床应用

摘  要:目的探讨下鼻甲部分切除术后免鼻腔填塞的方法。方法 168例符合下鼻甲部分切除患者行传统下鼻甲部分切除术,其中98例(观察组)以角型钛夹闭合鼻甲创面止血,70例(对照组)用凡士林油纱条或Merocel止血海绵填塞鼻腔止血。比较术后两组患者的鼻堵塞、出血量、咽喉干痛、头痛鼻痛、溢泪、睡眠状况、换药时患者疼痛等指标。结果观察组中发生术后出血量多6例,发生率为6.1%,对照组中术后抽取纱条时发生鼻出血量多4例,发生率为5.7%,两组比较差异无统计学意义(P>0.05);观察组患者术后的鼻堵塞、咽喉干痛等主观不适症状优于对照组,两组差异均有统计学意(P<0.05)。结论下鼻甲部分切除术后以角型钛夹闭合创面止血,免鼻腔填塞,避免鼻腔填塞带来的痛苦,可提高患者围手术期生活质量。Objective To discuss the method of avoiding nasal packing after partial inferior turbinectomy.Methods Conventional partial inferior turbinectomy were performed on 168 patients who needed partial inferior turbinectomy. Among the 168 cases, 98 patients(observation group) were treated with angle type titanium clip to stop turbinate wound bleeding, and 70 patients(control group) were treated with with vaseline gauze or Merocel hemostatic sponge packing to stop nasal bleeding. Postoperative nasal congestion, hemorrhage volume, dry throat pain,headache, nasal pain, epiphora, sleep status and dressing pain of the two groups were compared. Results There were 6 cases of postoperative bleeding(large amount) in observation group, with an incidence rate of 6.1%, and 4cases in the control group when extracting the gauze, with an incidence of 5.7%. There was no statistically significant difference between the two groups(P〉0.05). The discomforts of nasal congestion, dry throat pain and other subjective symptoms in observation group were better than those in the control group. The differences were statistically significant(P〈0.05). Conclusion Partial inferior turbinectomy combined with angle titanium clip wound hemostasis could avoid the pain of nasal packing and improve life quality of patients during the perioperation.
