肿瘤标志物在老年慢性肾脏病患者中的表达及相关因素分析

摘　　要：目的探讨非肿瘤老年慢性肾脏病患者是否存在血清肿瘤标志物水平的改变，并分析影响其改变的相关因素。方法随机选取复旦大学附属华东医院慢性肾脏病245例和健康体检者178例，比较两组血清肿瘤标志物水平的差异，并将慢性。肾脏病患者按。肾功能等级和年龄分组，比较各组间血清肿瘤标志物水平的差异。利用多因素Logistic回归分析了解老年慢性肾脏病患者血清肿瘤标志物水平的影响因素。结果慢性肾脏病患者组血清CA199、CA125、NSE水平高于健康对照组（P〈0．05）。血清CA199、NSE水平在不同估算肾小球滤过率（estimated glomerular filtration rate，eGFR）组间存在统计学差异（P〈0．05），血清CA199、CA724、CA211、CEA、PSA和fPSA水平在不同年龄组间存在统计学差异（p〈0．05）。血清CA125水平在不同eGFR组间差异无统计学意义（P〉0．05）。Logistic回归分析发现血清肿瘤标志物水平与患者的年龄、eGFR水平、血白蛋白水平和浆膜腔积液呈相关。结论老年慢性肾脏病患者存在部分肿瘤标志物水平的异常升高。在根据血清肿瘤标志物诊断肿瘤时，须结合患者年龄及肾功能情况等综合考虑。Objective To explore whether there are any changes of serum concentrations of tumor markers in patients with chronic kidney damage, and to analysis the related factors. Methods The data of 245 patients with chronic kidney damage and 178 normal persons from our hospital were analyzed to compare their serum concentrations of tumor markers. The patients with chronic kidney damage were divided into groups respectively according to their renal function grades and ages. The serum concentrations of tumor markers were analyzed and compared among the groups. Multiple regression analysis was used to identify risk factors associated with the levels of serum tumor markers. Results The serum levels of CA199, CA125, NSE were all significantly higher in patients with chronic kidney damage than in the control group （P〈0.05）. The serum levels of CA199, NSE in different eGFR groups and CA199, CA724, CA211, CEA, PSA, fPSA in different age groups had significant statistical differences （P〈0.05）. CA125 in different eGFR groups had no statistical differences （P 〉 0.05 ）. Multiple regression analysis confirmed significant relationship between the levels of tumor markers and age, eGFR, serum albumin and effusion in serous cavity. Conclusion The levels of tumor markers in patients with chronic kidney damage might be abnormally high so that age and renal function should be taken into account when di- agnosing malignant disease according to serum tumor markers.
