支气管肺泡灌洗术在小儿重症肺炎支原体肺炎患者抢救中的临床应用价值

摘　　要：目的探讨支气管肺泡灌洗术在小儿重症肺炎支原体肺炎患者抢救中的临床应用价值。方法选取2011年1月-2013年12月我院收治小儿重症肺炎支原体肺炎患者90例,根据患者不同治疗方法分为对照组以及实验组各45例,对照组患儿予以常规治疗,实验组患儿在常规治疗的基础上予以支气管肺泡灌洗治疗,观察两组患儿治疗的效果。结果治疗48 h后实验组患儿的Pa O2及氧合指数显著优于对照组,同时实验组Pa CO2显著低于对照组,差异有统计学意义（P〈0.01）;在治疗结果方面,对照组的有效率为68.9%,实验组为97.8%,差异有统计学意义（P〈0.01）。治疗前两组患儿的氧合指数无显著差异（P〉0.05）,治疗48 h后实验组患儿氧合指数显著高于对照组（P〈0.01）。结论支气管肺泡灌洗术治疗小儿重症肺炎支原体肺炎患者效果显著,值得在临床上推广应用。Objective To evaluate the clinical application value of bronchoalveolar lavage in the rescue of children with severe mycoplasma pneumonia. Methods Ninety children with severe mycoplasma pneumonia admitted to our hospital from January 2011 to December 2013 were selected and divided into the control group and the experimental group according to the treatment method, with 45 patients in each group. The control group was given conventional treatment and the experimental group was given bronchoalveolar lavage treatment on the basis of conventional treatment. The treatment effects of the two groups were observed. Results After 48 hours of treatment, the experimental group was significantly better than the control group in Pa O2 and oxygenation index and significantly lower than the control group in Pa CO2, with statistically significant differences（P〈0.01）; regarding to the treatment effects, the effective rate of the control group was 68.9% and that of the experimental was 97.8%, with statistically significant difference（P〈0.01）. Be fore treatment, the two groups were not significantly different in the oxygenation index（P〉0.05）, but after 48 hours of treatment, the oxygenation index of the experimental group was significantly higher than that of the control group（P〈0.01）.Conclusion Bronchoalveolar lavage shows remarkable effects in the treatment of children with severe mycoplasma pneumonia, thereby worthy of clinical promotion and application.
