后路椎弓根钉内固定联合PLIF治疗腰椎滑脱疗效分析

摘　　要：目的探讨后路椎弓根钉内固定联合椎间植骨融合(posterior lumbar interbody fusion,PLIF)治疗腰椎滑脱的临床疗效及安全性。方法将85例腰椎滑脱患者随机分为观察组43例及对照组42例,对照组采用后路滑脱复位椎弓根连接系统加椎间植骨融合治疗,观察组采用后路椎弓根钉内固定联合椎间植骨融合治疗,对比分析两组治疗效果。结果观察组滑脱融合率、总有效率、滑脱复位率高于对照组(P<0.05)。观察组手术时间、术中出血量、卧床时间、住院时间显著少于对照组(P<0.05)。观察组术后并发症发生率为11.63%,对照组为35.71%,差异有统计学意义(P<0.05)。结论后路椎弓根钉内固定联合PLIF治疗腰椎滑脱临床疗效显著,融合率、滑脱复位率高,术后并发症发生率低,有利于患者预后。Objective To explore the clinical efficacy and safety of pediele screw fixation combined PLIF treatment of lumbar spondylolisthesis. Methods 85 cases of spondylolisthesis were randomly divided into observation group （ n = 43 ） and control group （ n = 42 ）. The control group were treated with conventional surgery and the observation group were treated with pedicle screw fixation combined PLIF. The clinical efficacy of two groups was compared. Results The rate of pondylolisthesis fusion, the total efficiency, and spondyio- listhesis reduction of observation group were better than the control group （ P 〈 0.05）. The operative time, blood loss, time in bed and hospitalization time of observation groups were less than the control group （ P 〈 0.05）. The complication rate of the observation group was lower than the control group （ 11.63 % vs. 35.71% ）. Conclusion It shows significant clinical effect, high fusion rate and spondylolisthesis reduction rate, low inci- dence of postoperative complications and good prognosis of the posterior pediele screw fixation combined PLIF for lumbar spondylolisthesis patients.
