摘  要:目的探讨经颅多普勒超声(transcranial Doppler sonography,TCD)转颈试验在颈性眩晕诊断中的价值。方法选择头晕患者133例,其中颈性眩晕(观察组)67例,其他眩晕(对照组)66例。所有患者均行颈椎X线、MRI和椎动脉磁共振血管造影术以及TCD常规检查和TCD转颈试验。结果 2组患者的颈部外伤病史、骨质增生、颈椎曲度异常、颈椎不稳、椎间盘突出的异常百分比无显著差异,颈痛和磁共振血管造影术椎动脉受压百分比差异有统计学意义(P<0.05)。2组患者的常规TCD检测结果差异无统计学意义(P>0.05)。观察组患者TCD转颈试验阳性率为80.60%(54/67),对照组为4.55%(3/66),2组间差异有统计学意义(P<0.01)。结论 TCD转颈试验有助于颈性眩晕的诊断,可作为颈性眩晕诊断的一个客观指标。Objective To explore the value of the transcranial Doppler sonography( TCD) turning-neck tests on the diagnosis of the cervical vertigo. Methods A total of 133 patients who suffered vertigo were selected in this investigation,in which 67 patients suffered cervical vertigo in observe group and 66 others suffered other kinds of vertigo in control group,and all of them were tested by cervical X-ray,cervical MRI,magnetic resonance angiography( MRA) of vertebral artery,regular TCD test and TCD turning-neck tests. Results There had no prominent difference between 2 group in the abnormal percentage of neck injury history,hyperosteogeny,cervical curve abnormal,the unsteadiness of cervical vertebrae and lumbar disc herniation. The difference of the percentage of cervicodynia and vertebral artery compression showed by MRA between 2 group had statistical significance( P〈0. 05). The results of regular TCD test between 2 group had no difference( P〈0. 05). The positive rate of the TCD turning-neck tests in observe group was 80. 60%( 54 /67),while it was 4. 55%( 3 /66) in control group,and the difference had statistical significance( P〈0. 01). Conclusion The TCD turning-neck tests is helpful for the diagnosis of the cervical vertigo and will be one of objective indicators of the diagnosis.
