通督调神针刺法治疗强直性脊柱炎的疗效及与热像图的相关性

摘　　要：目的观察通督调神针刺法治疗强直性脊柱炎（AS）的临床疗效，并探讨其与热像图的相关性。方法强直性脊柱炎患者60例，采用通督调神针刺法治疗，治疗前后记录症状、体征，行红外热像图、实验室指标及影像学检查，并探讨热像图与CRP、ESR的相关性。结果治疗后，患者各症状及体征指标均较治疗前改善（P＜0．05）；治疗后ESR、CRP等炎性指标降低，较治疗前有统计学差异（P＜0．05）；红外热像图诊断As较HLA—B27，x线片及骶髂关节CT阳性率高（P＜0．05）；中心温度与周围组织温度之差（△T）与ESR和CRP呈正相关。结论通督调神针刺法治疗强直性脊柱炎患者疗效确切，且与红外热像图有较强的正性相关。Objective To observe the efficacy of acupuncture therapy （通督调神法） in the treatment of patients with ankylosing spondylitis （AS） and the correlation of thermal imaging with its effects. Methods Sixty patients with ankylosing spondylitis were selected and treated by acupunc- ture therapy （通督调神法）. Symptoms and signs, laboratory values and radiographic imaging were recorded before and after the treatment. The correlation between infrared thermography and ESR and CRP were analyzed respectively. Results There was a significant improvement in symptoms and signs of AS patients after the treatment （P〈0.05）. ESR, CRP and other inflammatory indexes were obviously decreased after the treatment （P〈 0.05）. The positive rate of infrared thermography （100%） was significantly higher than those of HLA-B27, X ray and CT for the diagnosis of AS （P 〈0.05）. Temperature difference （AT） was correlated positively with ESR and CRP （P〈0.05）. Conclusion Acupuncture therapy （通督调神法） is effective in the treatment of patients with ankylo- sing spondylitis, and there is a positive correlation between the efficacy and infrared thermal imaging.
