摘 要:目的:研究同种异体新鲜半月板移植重建兔肩关节前盂唇术后肩关节的前向稳定性。方法:10只成年大耳白兔构成前盂唇关节囊破坏组,制作肩关节前方不稳模型。于术后第12周及24周时各处死5只,切取双侧肩关节进行生物力学测试,比较双侧肩关节前向稳定性的差异。15只成年大耳白兔构成半月板移植组,切取其中1只的左膝内侧半月板,植入另1只的右肩关节,重建前盂唇。于术后第6周、12周、24周时各处死5只,切取双侧肩关节进行生物力学测试,比较双侧肩关节前向稳定性的差异。结果:(1)前盂唇关节囊破坏组:术后12周及24周时,试验侧肱骨头向前位移的距离明显大于对照侧,前盂唇关节囊组织的强度无明显差别。(2)半月板移植组:术后6周时,试验侧肱骨头向前位移的距离明显大于对照侧,前盂唇关节囊组织的强度明显低于对照侧。术后12周、24周时,试验侧肱骨头向前位移的距离与对照侧比较,差异没有统计学意义,前盂唇关节囊组织的强度与对照侧比较,差异没有统计学意义。结论:术后12周、24周,重建前盂唇后肩关节的稳定性与正常肩关节相同,其前盂唇关节囊组织的强度与正常肩关节无明显差异。Objective To study the biomechanical properties of the reconstructed shoulder anterior labrum with fresh allogenic meniscus in rabbit. Methods( 1 )Instability model was established on the right shoulder in 10 adult white rabbits by surgical procedure. Then the rabbits were sacrificed at week 12(n = 5)and 24 (n = 5),postoperatively. Both shoulders of each rabbit were resected to compare their anterior stability. (2)The reconstruction of anterior labrum was performed by transplantation of left knee medial meniscus of another adult white rabbit. Results (1)At week 12 and 24 postoperatively,anterior translation of humeral head in experimented side was significantly higher than that of the control side. The stiffness of anteroinferior lahrum and surrounding capsular ligament in experimented side was somewhat lower than that of the control side. (2)6 weeks after labrum reconstruction,anterior translation of humeral head in experimented side was significantly higher than that of the control side. The stiffness of anteroinferior labrum and capsular ligament in experimented shoulder was significantly lower than that of the control side. However, no significant differences of anterior translation of humeral head or the stiffness were found between the experimented shoulder and the control side at 12th and 24th week post-operation. Conclusion At 12th and 24th week after the transplantation,the stability of the experimented shoulder was equal to that of the control side, and no significant differences in the stiffness of anteroinferior labrum and capsular ligament were found between the experimented and the control sides.
