化痰祛瘀疏肝汤治疗痰瘀互阻型冠心病心绞痛随机平行对照研究

摘　　要：[目的]观察化痰祛瘀疏肝汤治疗痰瘀互阻型冠心病心绞痛疗效。[方法]使用随机平行对照方法,将60例门诊及住院患者按掷骰子法简单随机分为两组。对照组30例阿司匹林,100mg/次,1次/d;单硝酸异山梨酯,20mg/次,2次/d;急性疼痛发作舌下含化硝酸甘油,0.5mg/次。治疗组30例化痰祛瘀疏肝汤(瓜蒌30g,薤白、半夏各12g,枳实9g,赤芍、柴胡各12g,甘草炙6g,桂枝9g,丹参15g,砂仁、三七各6g;兼热加黄连6g,郁金10g,胆南星12g;兼水湿加荜茇9g,细辛3g;兼气虚加党参、黄芪各15g),1剂/d,水煎200m L,早晚口服。连续治疗28d为1疗程。观测临床症状、LDL-C、TG、TC、HDL-C、不良反应。治疗1疗程,判定疗效。[结果]心绞痛疗效:治疗组显效12例,有效15例,无效3例,加重0例,总有效率90.00%。对照组显效6例,有效14例,无效10例,加重0例,总有效率66.67%。治疗组疗效优于对照组(P<0.01)。心电图疗效:治疗组显效10例,有效15例,无效5例,加重0例,总有效率90.00%。对照组显效12例,有效6例,无效12例,加重0例,总有效率60.00%。治疗组疗效优于对照组(P<0.01)。血脂指标两组TG、TC、LDL-C均有改善(P<0.05,P<0.01),HDL-C无改善(P>0.05),治疗组改善优于对照组(P<0.05,P<0.01)。[结论]化痰祛瘀疏肝汤治疗痰瘀互阻型冠心病心绞痛效果显著,值得推广。[Objective]To observe Huatanquyu Shugan Decoction Angina. [Methods]Using random parallel control method,60 cases of outpatient and inpatient dice France by simple random into two groups. A control group of 30 patients aspirin,100 mg / times,1 time / d; isosorbide mononitrate sustained release tablets,20 mg / times,2 times / d; acute pain episodes sublingual nitroglycerin,0.5mg / times. Phlegm and blood stasis treated 30 cases of liver soup（Gualou 30 g,Xiebai,anxia 12 g,Zhishi 9g,Chishao,Chaihu each 12 g,Gancao 6g,Guizhi 9g,Danshen 15 g,Sharen,Sanqi each 6g;Heat plus Huanglian 6g,Yujin 10 g,Dannanxing12g; Piper longum and wet plus Biji 9g,Xixin 3g; and qi deficiency plus Dangshen,Huangqi 15g）,1/d,decoction 200 m L,sooner or later orally. 28 d is a continuous course of treatment. Observation of clinical symptoms,LDL-C,TG,TC,HDL-C,adverse reactions. A course of treatment to determine efficacy. [Results] angina pectoris： treatment group 12 cases,effective in 15 cases,3 cases,aggravated 0 cases,the total efficiency of 90.00%. A control group that were 6 cases,effective 14 cases,ineffective in 10 cases,aggravated 0 cases,the total efficiency of 66.67%. Treatmentgroup than the control group（P〈0.01）. ECG： treatment group 10 cases,effective 15 cases,ineffective in 5 cases,aggravated 0 cases,the total efficiency of 90.00%. Control group,12 cases markedly effective in 6 cases,12 cases,aggravated 0 cases,the total efficiency of 60.00%. Treatment group than the control group（P〈0.01）. Two sets of TG,TC,LDL-C were improved（P0.05,P0.01）,HDL-C,no improvement（P〈0.05）,to improve the treatment group than the control group（P0.05,P0.01）. [Conclusion]Liver soup Huatanquyu treatment of angina pectoris effect is significant,it is worth promoting.
