鼻息肉与上颌窦后鼻孔息肉变态反应差异的探讨

摘　　要：目的探讨鼻息肉（nasal polyps,NP）与上颌窦后鼻孔息肉（antrochoanal polyps,ACP）变态反应存在的差异,了解两种类型的鼻息肉疾病诱因和发病机制是否存在联系。方法选取笔者所在医院2011年1月-2014年1月所收治的42例NP及19例ACP患者相关资料进行分析,所有患者均实施变应原皮肤点刺实验（SPT）,了解两种类型的鼻息肉疾病诱因和发病机制是否存在联系。结果 42例NP患者中20例出现阳性反应,占47.6%。3例单一变应原阳性,17例2种以上变应原阳性。19例ACP患者中6例出现阳性反应,占31.6%,2例单一变应原反应,共9种变应原出现阳性;44种测试液中24种并未发生过敏现象,且上颌窦后鼻孔息肉组患者阴性率为68.4%,高于鼻息肉组阴性率（52.4%）（P〈0.05）。结论鼻息肉和上颌窦后鼻孔息肉变态反应方面存在较大的差异,两种疾病诱因以及疾病的发病机制之间存在明显的区别。Objective To explore the differences of allergic reactions between nasal polyps（NP） and antrochoanal polyps（ACP）, and to understand whether there is a connection between diseases causes and pathogenesis in the two types of polyps. Methods A total of 42 patients with NP and 19 patients with ACP in our hospital from January 2011 to January 2014 were selected. All patients received skin prick test（SPT）, in a way to understand whether there was a connection between disease causes and pathogenesis in the two types of polyps. Results 20 cases of 42 patients with NP showed positive reactions, accounting for 47.6%, among them 3 patients were positive in single allergen and 17 patients were positive in two or more allergens. 6 of 19 patients with ACP showed positive reactions, accounting for 31.6%, among them 2 patients were positive in single allergen. 9 allergens altogether showed positive in patients; 24 out of 44 types of test fluid had never shown any allergic reactions, and the negative rate of patients with ACP was 68.4%, higher than that of 52.4% in patients with NP（P〈0.05）. Conclusion There is a relatively big difference in allergic reactions between NP and ACP, proving that the disease causes and pathogenesis of the two diseases have distinct differences.
