胫骨平台骨折合并交叉韧带损伤的早期诊断

摘　　要：探讨胫骨平台骨折合并交叉韧带损伤时 ,避免交叉韧带损伤漏诊的方法和意义。方法 :2 8例胫骨平台骨折在内固定手术中 ,分别于麻醉后或内固定后行膝关节稳定试验 ,呈阳性者在内固定术同时行膝关节探查及交叉韧带修补术。 2 8例术中抽屉试验均为阳性 ,与探查结果完全符合。术后 7～ 5 4个月随访。结果 :所有病例膝关节稳定性良好 ,抽屉试验阴性 ,未见创伤性关节炎发生。结论 :对有内固定手术指征的胫骨平台骨折 ,术中作膝关节稳定试验以决定是否行膝关节探查 ,是避免交叉韧带损伤漏诊的一个可靠而简便的方法。Objective:To investigate the methods and significance to avoid failure to diagnosis cruciate ligament injuries assiociated with fractures of the tibial plateau.Methods:Each of 28 patients with fractures of the tibial plateau had a knee stability test under anesthesia or after internal fixation within operation, if positive,a exploration of knee joint and repair or reconstruction of cruciate ligament should be performed at the same time.Results:The positive rate of knee stability test was 100% in all 28 patients. After an average of 20.6 months followed-up ,knee stability test was negative,and there was satisfactory stability of knee joint and no traumatic arthrosis in all cases.Conclusion: For patients with fractures of the tibial plateau indicated for an internal fixation, a knee stability test and possible exploration of knee joint within operation is an effective and simple method to avoid failure to diagnosis cruciate ligament injuries.
