摘　　要：目的观察鼻内镜下单纯腺样体切除术与鼻内镜下腺样体切除加鼓膜置管术治疗儿童分泌性中耳炎(SOM)伴腺样体肥大的疗效。方法将60例SOM伴腺样体肥大患儿随机分为2组,试验组行鼻内镜下腺样体切除术,对照组行鼻内镜下腺样体切除加鼓膜置管术。2组术后均跟踪随访,观察并比较2组患儿的临床疗效。结果治疗后4个月,试验组总有效率为92.98%,对照组总有效率为93.10%,2组比较差异无统计学意义(P>0.05)。结论对于儿童SOM伴腺样体肥大,鼻内镜下腺样体切除术与腺样体切除加鼓膜置管术均是有效的治疗方法,但鼻内镜下腺样体切除术更具有优势,值得临床推广和应用。Objective To observe the curative effect of transnasal endoscopic pure adenoidectomy and transnasal endoscopic adenoidectomy combined with tympanostomy tube insertion on treating secretory otitis media （ SOM ） in children with adenoidal hypertrophy .Methods Sixty cases of SOM with adenoidal hypertrophy were randomly divided into two groups .The study group underwent transnasal endoscopic pure adenoidectomy , while the control group underwent transnasal endoscopic adenoidectomy combined with tympanostomy tube insertion .All cases were followed up and the curative effects in two groups were observed .Results After four months of operation , the total effective rate of study group was 92.98%, while the total effective rate of control group was 93.10%.No statistical difference existed between the two groups（P〉0.05）.Conclusions Both transnasal endoscopic pure adenoidectomy and transnasal endoscopic adenoidectomy combined with tympanostomy tube insertion are effective treatment methods for SOM with adenoidal hypertrophy, but transnasal endoscopic pure adenoidectomy has more superiority , and is worthy of promotion and application.
