中医药治疗中晚期食管癌疗效的Meta分析

摘　　要：[目的]系统评价中医药治疗中晚期食管癌的疗效及不良反应,为中晚期食管癌的中医药治疗提供依据。[方法]全面检索2004年1月1日至2014年5月1日中医药联合化疗治疗中晚期食管癌的临床试验文献,按照纳入标准对文献进行筛选,对单纯化疗作为对照组,化疗加中医药治疗作为治疗组的随机对照试验进行Meta分析。[结果]共纳入12个试验。治疗组和对照组的总例数分别为541例和424例。结果显示:治疗组比对照组能明显增加近期疗效,改善生活质量,提高1年生存率,减少转氨酶升高、肌酐升高、白细胞下降、血小板下降、恶心呕吐等不良反应,差异具有统计学意义(P<0.05)。但在降低神经毒性发生方面,差异无统计学意义(P>0.05)。[结论]化疗加用中医药治疗中晚期食管癌疗效优于单纯化疗,且能明显改善化疗后的生活质量,延长生存期,减少化疗后的毒副反应及肝肾损害。[Objective] Systematic evaluation of the effect and adverse reactions of traditional Chinese medicine in treating middle-advanced esophageal carcinoma, and provide therapy evidence for middle-advanced esophageal carcinoma. [Methods] Generally retrieved clinical literatures of TCM treating middle- advanced esophageal carcinoma, selected literatures according to inclusion criteria, conducted simple chemotherapy as control group and chemotherapy plus TCM as treatment group, had a comparison by meta-analysis. [Results] Total 12 experimental studies were brought into. Total cases in treatment and control groups were 541 and 424. Meta-analysis indicated that treatment group were superior to control group in improving the short term effect and the quality of life and increasing one year survival rate, decreasing the incidence of transaminase elevation, the incidence of creatinine elevation, inhibiting the leukocyte decrease, and the platelet descended, improving the symptoms of nausea and vomiting, there were significant statistical difference between two groups. But in decreasing the incidence of neurotoxicity, there was no significant statistical difference between two groups.[Conclusion] The curative effect of chemotherapy plus TCM is superior to the simple chemotherapy, and can significantly improve the quality of life after chemotherapy, prolong survival time, reduce the noxious side effect of chemotherapy, and reduce the hepatorenal damage.
