超声弹性成像技术在2型糖尿病合并高血压患者动脉血管壁弹性评估中的价值

摘　　要：目的探讨超声弹性成像技术在2型糖尿病(T2DM)合并高血压(EH)患者动脉血管壁弹性评估中的价值。方法 72例T2DM患者根据是否合并高血压分为T2DM组(n=36例)和T2DM+EH组(n=36例),同期选择35例门诊健康体检者作为研究对照组。比较各组颈、肱动脉弹性功能参数,分析脉压、血糖、IMT厚度与颈、肱动脉B/A的相关性。结果 T2DM+EH组和T2DM组患者颈动脉IMT厚度明显高于对照组(P<0.05);T2DM+EH组和T2DM组患者颈、肱动脉B/A明显高于对照组(P<0.05),且T2DM+EH组较T2DM组更高(P<0.05);颈动脉B/A与脉压、IMT厚度呈正相关性关系(P<0.05);颈动脉IMT厚度与脉压、血糖水平呈正相关性关系(P<0.05);肱动脉B/A1与血糖水平呈正相关性关系(P<0.05)。结论超声弹性成像技术可快速和有效评价T2DM+EH患者早期动脉血管壁弹性变化。Objective To investigate the value of ultrasonic elastography technique on assessment of artery wall elasticity in type 2 diabetes patients （T2DM） with hypertension. Methods 72 cases of T2DM patients were divided into T2DM group （n=36 cases） and T2DM＋EH group （n= 36 cases） according to whether they suffered from hypertension, at meantime, 35 healthy persons were selected over the same period as control group. The elastic function parameters of carotid, braehial artery were compared and the correlation among pulse pressure, blood glucose, IMT thickness and carotid, and braehial artery B/A were analyzed. Results The IMT thickness of carotid artery in T2DM＋EH group and T2DM group was higher than that of control group （ P〈0.05）5 carotid and brachial artery B/A of T2DM＋ EH group and T2DM group was significantly higher than those of the control group （ P 〈0.05）, and carotid and brachial artery B/A of T2DM＋EH group was significantly higher than that of T2DM group （ P〈0.05） ; The carotid artery B/A had positive relation with pulse pressure and IMT thickness （ P〈0.05） ; the IMT thickness carotid artery was positively related to pulse pressure and blood glucose levels （ P〈0.05） ; the brachial artery B/Al had positive relation with blood glucose levels （ P 〈0.05）. Conclusion Ultrasonic elastography technique can quickly and effectively evaluate artery wall elasticity changes on patients with T2DM＋ EH.
