摘　　要：目的探讨椎间盘镜下减压B-Twin椎间植骨融合术治疗高位腰椎间盘突出症的临床疗效。方法自2008-01—2010-05收治高位腰椎间盘突出症32例,均为单节段病变,L1、24例,L2、38例,L3、420例。均行后路椎间盘镜下B-Twin椎间植骨融合术。结果 31例获得随访,随访时间13-36个月,平均26.7个月。术后腰腿疼痛症状均明显缓解或消失,VAS评分术前平均为（8.5±1.2）分,末次随访平均为（0.5±0.4）分;ODI术前平均（79±4）%,末次随访为（15±4）%,术后各时间点与术前比较差异有统计学意义（P〈0.01）;椎间隙高度术前平均为（8.95±1.4）mm,术后12个月平均为（12.2±0.7）mm。31例椎间植骨均获得融合,融合时间为4.1-6.1个月,平均4.9个月。结论椎间盘镜下B-Twin椎间植骨融合术治疗高位腰椎间盘突出症具有创伤小、临床疗效可靠的优点。Objective To explore the effect of B-Twin intervertebral cage in treatment of high level lumbar disc herniation by mircroendoscopic discectomy. Methods From January 2008 to May 2010, 32 patients with high level lumbar disc herniation were treated with B-Twin intervertebral cage by mircroendoscopic discectomy. All the cases suffered from only one level disc protrusion, L1、24 cases, L2、38 cases,and L3、420 cases. Results Thirty one patients were followed up for an average of 26.7 months（range, 13-36）. All the patients＇ low back pain and radiation pain disappeared or were relieved apparently. The mean preoperative ODI and VAS scores decreased from（79 ±4）% to（15 ±4）%, and（8.5 ±1.2） to（0.5 ±0.4） at the final follow-up respectively. Disc space increased from a pre-operative height of（8.95 ±1.4）mm to a height of（12.2 ±0.7） mm 12 month postoperatively. Thirty one patients achieved solid union at a mean time of 4.9 months（range, 4.1-6.1）. Conclusion Posterior lumbar interbody fusion using B-Twin intervertebral cage by mircroendoscopic discectomy can obtain satisfactory outcomes in the treatment of high level lumbar disc herniation.
