摘  要:目的观察右美托咪定对患者红细胞醛糖还原酶(AR)活性及血浆丙二醛(MDA)和皮质醇(Cor)水平的影响。方法 40例食管癌根治术患者随机均分为两组:D组于麻醉诱导时15min内静脉泵入右美托咪定1μg/kg,继以0.2-0.7μg·kg-1·h-1维持;S组给予等容积生理盐水作对照。分别于麻醉前30min(T0)、术毕1h(T1)、术后1d(T2)、2d(T3)和3d(T4)抽取静脉血测定红细胞AR活性及血浆MDA和Cor的浓度。结果与T0时相比,T1、T2时两组红细胞AR活性及血浆MDA和Cor浓度升高(P<0.05)。与S组相比,T1时D组红细胞AR活性及血浆MDA和Cor浓度降低(P<0.05)。结论食管癌根治术患者围术期红细胞多元醇通路被激活,氧化应激反应增强,持续泵注右美托咪定能减轻这一反应。Objective To investigate the effects of dexmedetomidine on the activity of erythrocyte aldose reductase(AR) and plasma levels of malondialdehyde(MDA) and cortisol(Cor). Methods Forty patients undergoing radical surgery for esophageal cancer were equally randomized into groups of D and S. The patients in group D were given a loading dose of dexmedetomidine 1 ~g/kg intravenously within 15 minutes before anesthesia induction, which was followed by continuous infusion of dexmedetomidine 0. 2-0.7μg·kg-1·h-1. The patients in group S received the same volume of normal saline as the controls. Venous blood samples were collected for the measurements of erythrocyte AIR activity and plasma MDA and Cor before anesthesia induction(T0), at 1 h after the end of the operation(T1) and on the 1st (T2), 2nd (T3) and 3rd (T4) postoperative day. Results Compared with TO, AR activity and plasma levels of MDA and Cot were inereased at T1 and T2 in two groups(P〈0. 05). Compared with group S,AR activity and plasma levels of MDA and Cor at T1 were decreased in group D (P〈0. 05 ). Conclusion A continous intravenous infusion of dexmedetomidine is able to attenuate AR activity on the polyol way of glucose metabolism in erythrocyte and decrease the oxidative stress response in the patients undergoing radical surgery for esophageal cancer.
