冠脉造影术中血压升高患者血管内皮功能与冠脉狭窄的相关性

摘　　要：目的:探讨冠脉造影术中血压升高的非高血压患者血管内皮功能的变化与冠脉狭窄的相关性。方法:选择990例疑诊冠心病的非高血压患者,将冠脉造影术中血压≥140/90mmHg者分为观察组,而血压<140/90mmHg为对照组,测定一氧化氮(NO)、肱动脉介导的血管内皮功能(FMD)及血管性假血友病因子(vWF)和单核细胞趋化蛋白-1(MCP-1)。结果:1观察组狭窄>50%者显著高于对照组,而对照组冠脉造影正常者显著高于观察组。2观察组NO、FMD显著低于对照组,而vWF和MCP-1显著高于对照组。3随访3年,观察组诊断高血压者显著高于对照组。结论:对于冠脉造影术中血压升高的非高血压患者,其冠状动脉狭窄与内皮功能障碍有关,此类患者发展为高血压的风险显著升高。Objective：To investigate the correlation between endothelial function and coronary artery stenosis of non-hypertensive patients with elevated blood pressure in coronary artery angiography.Methods：A total of 990non-hypertensive patients with suspected coronary heart disease were selected.Those with blood pressure≥140/90 mmHg were separated into observation group,while those with blood pressure140/90 mmHg were separated into control group.Levels of nitric oxide（NO）,brachial artery flow-mediated dilation（FMD）,von wilebrand factor（vWF）and monocyte chemoattractant protein（MCP-1）were determined.Blood pressures were observed during 3years of follow-up.Results：There were more patients with50%stenosis in observation group than those in control group,while there were less patients with normal coronary artery angiography in control group than those in observation group.NO and FMD were lower but vWF and MCP-1were higher in observation group than those in control group.More patients were diagnosed to be hypertension in observation group than those in control group during3 years of follow-up.Conclusion：The coronary artery stenosis is correlated to endothelial dysfunction of non-hypertensive patients with elevated blood pressure in coronary artery angiography who have the high risk of developing hypertension.
