摘 要:目的探讨尿脂质运载蛋白2(u Ln-2)在狼疮性肾炎(LN)患者早期诊断中的临床意义。方法选取2010年3月至2013年9月内蒙古自治区人民医院肾内科收治的107例LN患者为研究对象,根据尿液中微量白蛋白(m Alb)水平分为Ⅰ组(m Alb〈20 mg/L,33例)、Ⅱ组(m Alb 20~200 mg/L,35例)和Ⅲ组(m Alb〉200 mg/L,39例),同期选取门诊体检的40例健康体检者作为对照组,测定其u Ln-2、血肌酐、血尿素氮(BUN)、血清胱抑素C(Cys-C)水平。结果 u Ln-2水平在对照组、Ⅰ组、Ⅱ组、Ⅲ组逐渐增高,组间比较差异有统计学意义[分别为(7.2±0.7)、(9.7±0.8)、(12.2±0.9)、(15.0±1.0)μg/L,P〈0.05];Ⅲ组血肌酐[(80±13)μmol/L]、BUN[(12±4)mmol/L]、Cys-C[(1.55±0.54)mg/L]明显高于Ⅰ组[(65±12)μmol/L、(6±2)mmol/L、(0.90±0.22)mg/L]、Ⅱ组[(70±13)μmol/L、(7±3)mmol/L、(1.02±0.30)mg/L]和对照组[(67±11)μmol/L、(6±2)mmol/L、(0.87±0.20)mg/L],差异均有统计学意义(P〈0.05),但在对照组、Ⅰ组、Ⅱ组之间比较差异无统计学意义(P〉0.05);LN患者的u Ln-2与m Alb之间呈正相关性(r=0.854,P〈0.05)。结论 u Ln-2水平是检测LN患者早期肾功能损伤的敏感、有效的指标,且随着肾脏损伤程度的加重持续增高。Objective To investigate the clinical significance of urinary lipid carrier protein-2( urinary lipocalin-2,u Ln-2) in early diagnosis of lupus nephritis( LN). Methods A total of 107 patients with LN admitted in People's Hospital of the Inner Mongolia Autonomous Region were divided into group Ⅰ,ⅡandⅢaccording to the levels of microalbumin( m Alb) in urine: group Ⅰ( m Alb 20 mg /L,n = 33),group Ⅱ( m Alb 20-200 mg/L,n = 35) and group Ⅲ( m Alb 200 mg/L,n = 39). Another 40 cases of healthy people underwent physical examination during the same period were selected as the control group. Levels of u Ln-2,serum creatinine,blood urea nitrogen( BUN),cystatin-C( Cys-C) of all the groups were measured. Results The levels of u Ln-2 in the control group,groupⅠ,groupⅡ and groupⅢ increased successively,the difference between either two groups was statistically significant [( 7. 2 ± 0. 7) μg / L vs( 9. 7 ± 0. 8) μg / L vs( 12. 2 ± 0. 9) μg/L vs( 15. 0 ±1. 0) μg/L,P〈0. 05]; the levels of serum creatinine [( 80 ±13) μmol/L],BUN[( 12 ± 4) mmol / L],Cys-C[( 1. 55 ± 0. 54) mg / L] of group Ⅲ were significantly higher than those in the control group[serum creatinine,BUN,Cys-c were( 67 ± 11) μmol / L,( 6 ± 2) mmol / L,( 0. 87 ±0. 20) mg / L,respectively ],and those in group Ⅰ [( 65 ± 12) μmol / L,( 6 ± 2) mmol / L,( 0. 90 ±0. 22) mg / L],as well as group Ⅱ [( 70 ± 13) μmol / L,( 7 ± 3) mmol / L,( 1. 02 ± 0. 30) mg / L,P 0. 05 ],there was statistically significant difference between any of the groups and group Ⅲ,but there were no statistically significant differences of the above indicators between either two of the control group,groupⅠ and groupⅡ( P〈0. 05); the levels of u Ln-2 in patients with LN were positively correlated with m Alb( r = 0. 854,P 0. 05).
