摘  要:目的观察丹红注射液治疗冠状动脉微血管功能障碍的效果。方法入选有胸痛症状、冠脉造影无显著狭窄并且满足运动平板试验阳性或冠脉慢血流诊断标准的患者共84例,分为两组。治疗组给予丹红注射液40m L静脉滴注14d。治疗前后检测腺苷负荷后的冠脉舒张期峰值血流速度(DPV),评价冠脉血流储备(CFR),记录运动平板试验中累积最大ST段偏移程度并测定血清内皮素-1(ET-1)浓度。结果两组治疗前DPV、CFR、累积最大ST段偏移和ET-1浓度均无统计学差异(P>0.05);治疗14d后,丹红组腺苷负荷后DPV及CFR显著高于对照组(P<0.05),丹红组的运动平板试验中累积最大ST段偏移显著低于对照组(P<0.05),丹红组的血清ET-1水平显著低于对照组(P<0.05)。结论丹红注射液能有效改善冠状动脉微血管功能障碍患者冠脉血流速度及血流储备,提高患者对负荷诱发的缺血耐受,该疗效与降低血清ET-1水平有关。Objective To investigate the efficacy of Danhong injection in treatment of coronary microvascular dysfunction( CMD).Methods 84 patients with chest pain,but in absence of obstructive CAD confirmed by quantifying coronary angiography met inclusion criteria of positive treadmill exercise testing or coronary slow flow documented by corrected TIMI frame count,were assigned randomly to two groups,and received transthoracic Doppler echocardiography measurement of distal left anterior descending at rest and adenosine infusion induced hyperemia state. CFR was calculated as the ratio of maximal hyperemia and baseline diastolic peak flow velocity before and after treatment. Cumulative maximal ST- segment deviation extensions in exercise stress testing and serum endothelin- 1 concentrations were measured concurrently. Results There were no significant differences between the 2 groups at baseline. After treating for 14 days,diastolic peak flow velocity after adenosine infusion and CFR were higher in patients with Danhong injection treatment than in patients of control arm( P 0. 05).Danhong injection was observed to decrease cumulative maximal ST- segment deviation extensions( P 0. 05) and serum endothelin- 1 concentrations( P 0. 05). Conclusion In patients with CMD,treating with Danhong injection is associated with improvement of coronary flow hemodynamics and tolerance to stress induced myocardial ischemia. The beneficial effects could be related to its effect on lowering ET-1level.
