逍遥散加减调节卒中后抑郁症临床研究

摘　　要：目的观察细胞凋亡、调控基因与卒中后抑郁症(PSD)的相关性及中药对PSD的调节作用。方法将51例PSD患者予逍遥散加减治疗21 d,治疗前后分别检测淋巴细胞凋亡率及B淋巴细胞瘤-2基因(Bcl-2)表达情况,并与健康对照组40例进行比较;统计治疗组治疗前后症状改善情况。结果治疗组治疗前淋巴细胞凋亡率及Bcl-2表达明显高于健康对照组(P<0.01),治疗后较治疗前明显降低(P<0.05)。治疗组治疗后除淋巴结肿痛、嗳气、呃逆、咳嗽、恶心、呕吐、便秘、吐酸、嘈杂、口干、口渴等症状外,其余症状较治疗前均有明显改善(P<0.01,P<0.05)。结论 PSD的发病过程存在着细胞凋亡,且中药治疗有一定调节作用。Objective To study the correlation of the cell apoptosis, regulation gene and post - stroke de- pression（PSD） and regulating effeet of Chinese traditional medicine on PSD. Methods 51 PSD patients （treatment group） were treated by Xiaoyaosan for 21 days. Lymphoid cell apoptosis rate and the Bel -2 expression detected be- fore and after treatment. The results were compared with those in healthy control group of 40 cases. The improvement before and after treatment were analyzed in treatment group. Results Lymphoid cell apoptosis rate and the Bcl - 2 expression before treatment in treatment group was siguificantly higher than those in healthy control group （P 〈 0.01 ）. Lymphoid cell apoptosis rate and the Bcl - 2 expression after treatment in treatment group were significantly decreased（P 〈 0.05 ）. Clinical symptoms after treatment in treatment group were improved besides Lymph node swell- ing pain, belching, hiccups, cough, nausea, vomiting, constipation, noisy, dry mouth, thirst, spit acid （ P 〈 0. 01 , P 〈 0.05 ）. Conclusion There is a cell apoptosis in process of PSD with stagnation of liver qi and spleen deficiency and Chinese herbal medicine treatment has a certain adjustment.
