摘  要:目的探讨血小板(PLT)、血红蛋白(Hb)、血浆纤维蛋白原(Fbg)及D二聚体(D-D)检测对产后出血的预测价值。方法选取2013年1月-2014年8月在我院门诊按期产检并在本院分娩的产妇,产前行PLT、Hb、Fbg及DD检测并观察产后出血情况。结果正常产妇组PLT和Fbg均高于产后出血组和严重出血组,而D-D低于产后出血组和严重出血组,差异有统计学意义(P〈0.05);产妇产前(孕33~34周)Fbg水平与产后出血量之间呈负相关(r=-0.463,P〈0.05),产前(孕33~34周)D-D水平与产后出血量之间呈正相关(r=0.482,P〈0.05),而PLT与产后出血量之间无相关性(P〉0.05)。结论产前(孕33~34周)检测Fbg及D-D对孕妇产后出血有一定的预测价值,而检测PLT和Hb预测价值不明确。Objective To investigate platelet( PLT),hemoglobin( Hb),plasma fibrinogen( Fbg) and D-dimer( D-D) detection of the predictive value of postpartum hemorrhage. Methods From 2013 January-to2014 August in our hospital out-patient regular examination and in the hospital birth mothers,prenatal PLT,Hb,Fbg and D-D to detect and observe the postpartum hemorrhage. Results Normal pregnant group PLT and Fbg were higher than that of postpartum hemorrhage group and severe hemorrhage group,and D-D was lower than that of postpartum hemorrhage group and severe hemorrhage group,the difference was statistically significant( P 〈0. 05); Maternal prenatal( 33 to 34 weeks gestation) the level of Fbg was negatively correlated with postpartum hemorrhage volume( r =-0. 463,P 〈0. 05),prenatal( 33 to 34 weeks gestation) D-D level was positively correlated with postpartum hemorrhage volume( r = 0. 482,P 〈0. 05),while PLT and no correlation between the amount of postpartum hemorrhage( P 〈0. 05). Conclusion Prenatal( 33 to 34 weeks gestation) Fbg and DD for pregnant women to detect postpartum hemorrhage have some predictive value,and detect PLT and Hb predictive value is not clear.
