摘  要:目的探讨γ-干扰素释放试验在肺结核诊断中的应用价值。方法用γ-干扰素释放试验试剂盒(WT-IGRA)定量检测65例肺结核病患者、45例非肺结核患者和20例体检健康者外周血结核特异性γ-干扰素的含量,同时与结核菌素纯化蛋白质衍生物(PPD)试验及结核蛋白芯片(TB-Ig G)检测进行平行比较分析。结果 WT-IGRA的敏感度、特异度、阳性预测值、阴性预测值均为89.23%,Youden指数78.46%,ROC曲线下面积0.89,诊断价值优于PPD试验及TB-Ig G检测。结论 WTIGRA的应用将有助于提高肺结核感染的阳性检出率,可作为临床肺结核诊断重要的辅助手段之一。Objective To investigate the clinical application of IFN-γ release test in diagnosis of pulmonary tuberculosis. Methods The concentration of mycobacterium tuberculosis specific IFN-γ in peripheral blood of 65 patients with pulmonary tuberculosis,45 patients with non- tuberculosis and 20 cases of normal physical examination were detected by the IFN-γ release test reagents( WTIGRA),and the results were parallel comparatively analyzed with purified protein derivative of tuberculin( PPD) test and tuberculosis protein chip( TB-Ig G). Results The specificity,sensitivity,positive predictive value,negative predictive value and of WT-IGRA were all 89. 23%,Youden index was 78. 46%,and the following ROC curve was 0. 89. The diagnostic value of WT-IGRA was better than the PPD test and TB-Ig G test. Conclusion WT-IGRA can improve the positive rate of tuberculosis infection diagnosis,and can be used as an important clinical auxiliary diagnosis for pulmonary tuberculosis.
