兔前交叉韧带不同分束断裂对内侧半月板的影响

摘　　要：目的探讨兔前交叉韧带(ACL)不同分束断裂对内侧半月板的影响。方法 36只新西兰大白兔随机分为ACL前内侧束(AMB)断裂组、后外侧束(PLB)断裂组及未断裂组,每组12只。分别制备兔右后肢AMB断裂、PLB断裂及未断裂模型,取第4、6、8周为观测点,记录兔术侧膝关节功能评分,取术侧半月板行组织学切片苏木精-伊红染色,免疫组织化学法检测半月板中金属基质蛋白酶13(MMP-13)表达情况。结果术后4、6、8周未断裂组兔术侧半月板胶原纤维、软骨细胞形态正常,无血管增生,无炎性细胞,胶原纤维束粗大,排列整齐;AMB断裂组及PLB断裂组兔术侧半月板表面逐渐出现不平整、组织疏松,胶原纤维排列紊乱,炎性细胞浸润。术后4、6、8周未断裂组兔术侧膝关节功能评分差异无统计学意义(P>0.05);ABM断裂组和PLB断裂组兔术后6、8周的术侧膝关节功能评分均低于4周时(P<0.05),8周的术侧膝关节功能评分均低于6周时(P<0.05);同时间点比较,AMB断裂组和PLB断裂组兔术侧膝关节功能评分均较未断裂组显著降低(P<0.05);AMB断裂组与PLB断裂组同时间点比较术侧膝关节功能评分差异无统计学意义(P>0.05)。未断裂组兔术后第4、6、8周半月板MMP-13阳性细胞百分率差异无统计学意义(P>0.05)。AMB断裂组、PLB断裂组兔术后6、8周的MMP-13阳性细胞百分率均高于4周时(P<0.05),8周的MMP-13阳性细胞百分率均高于6周时(P<0.05)。相同时间点AMB断裂组、PLB断裂组兔MMP-13阳性细胞百分率均高于未断裂组(P<0.05),相同时间点AMB断裂组兔MMP-13阳性细胞百分率高于PLB断裂组(P<0.05)。结论兔前交叉韧带不同分束断裂均可引起内侧半月板的退变,AMB断裂比PLB断裂更易引起内侧半月板退变。Objective To investigate the influence of different bundle fracture of anterior cruciate ligaent on the medial meniscus in rabbits. Methods Thirty-six New Zealand rabbits were randomly divided into anteromedial bundle（ AMB） fracture group,posterolateral bundle（ PLB） fracture group and no fracture group,with 12 rabbits in each group. The right legs were used to make AMB fracture models,PLB fracture models and no fracture models respective. The 4th,6thand 8thweeks of observation point were taken to record the knee behavioral score of the operation side in experimental animals. The meniscus histological section of operation side was taken for haematoxylin-eosin stain. The percentage of matrix metalloproteinase-13（ MMP-13） was assayed by immunohistochemical staining. Results At the 4th,6thand 8thweek after operation,the rabbits in no fracture group showed normal collagen fiber and cartilage cells form,no blood vessels and inflammatory cell,collagen fiber bundle was thick,arranged in order. The rabbits in AMB fracture group and PLB fracture group showed uneven surface of meniscus,loose tissue,disorder of collagen fiber,infiltration of inflammatory cell. At the 4th,6thand 8thweek after operation,the knee behavioral scores in no fracture group showed no statistical significant（ P〉0. 05）,In AMB fracture group and PLB fracture group,the knee behavioral score at the 6thand 8thweek was significantly lower than that at the 4thweek（ P〈0. 05）; the knee behavioral score at the 8thweek was significantly lower than that at the 6thweek（ P〈0. 05）. At the same observation time point,the knee behavioral score in AMB fracture group and PLB fracture group was significantly lower than that in no fracture group（ P〈0. 05）,the knee behavioral score between AMB fracture group and PLB fracture group showed no statistical significance（ P〉0. 05）. At the 4th,6thand 8thweek,the percentage of positive cell of MMP-13 had no significant change in no fracture group（ P〉0. 05）. In AMB fracture group and
