自行调节式口腔矫治器治疗OSAHS的临床疗效评价

摘　　要：目的研究自行调节式口腔矫治器治疗OSAHS的临床疗效。方法用ESS量表、Berlin睡眠症状调查问卷及多导睡眠监测(PSG)对56例经PSG监测诊断为轻、中度的OSAHS患者戴用自行调节式口腔矫治器治疗前后的主观、客观疗效进行评价。结果 56例OSAHS患者戴用自行调节式口腔矫治器3个月后,ESS量表评分及Berlin睡眠症状调查问卷评分均较3个月前明显降低;PSG监测指标(呼吸紊乱指数AHI、呼吸暂停指数AI、低通气指数HI、最低血氧饱和度(SPO2)、最长呼吸暂停时间与3个月前相比较,差异有统计学意义(P<0.05)。结论自行调节式口腔矫治器可有效减轻OSAHS患者的白天嗜睡、打鼾等主观症状,改善患者睡眠过程中的呼吸情况,使OSAHS患者的睡眠呼吸状况趋向于健康正常人。Objective To evaluate the clinical effects of the self- adjustive oral appliance in treating in the obstructive sleep apnea and hypopnea syndrome（ OSAHS） patients. Methods 56 patients who were diagnosed of mild to moderate OSAHS by PSG were analyzed,and the subjective and objective effects of the self- adjustive oral appliance were evaluated by using ESS scale,Berlin Sleep symptoms questionnaire and PSG. Results In the OSAHS patients treated with self- adjustable oral appliance for three months,the ESS score and Berlin sleep symptom questionnaire score were significantly lower than before. Also,the PSG monitoring indicators（ the respiratory disturbance index,AHI,apnea hypopnea index（ AI）,hypopnea index（ HI）,the lowest oxygen saturation（ Sp O2）,the longest apnea time） improved significantly（ P 〈0. 05）. Conclusion The self- adjustable oral appliance can be effective in the treatment of OSAHS patients by improving the subjective symptoms such as daytime sleepiness and snoring,breathing during sleep,thus,enhancing the recovery.
