我院门急诊处方退药管理方案的建立与实施

摘　　要：目的:加强医院处方退药管理。方法:制定退药标准、不予退药标准、退药流程及退回药品的处置等退药相关管理制度并实施,通过比较实施前(2008年至2009年6月)、后(2009年7月至2013年)我院的退药金额以评价实施效果并提出相关建议。结果与结论:通过建立详细的退药管理制度并将退药金额与退药科室的成本相结合的方法,我院门急诊处方退药金额由2008年的788万元下降至2013年的9万元。除建立合理的退药制度并实施相应的管理方法外,同时建议国家食品药品监督管理总局及医院建立信息统一的药品说明书分类数据库、药师在信息化基础上及时审核医师处方等,以便有效地减少退药现象的发生。OBJECTIVE： To strengthen the management of prescription drug withdrawal in the hospitals. METHODS： Some re- lated drug withdrawal management systems were determined and adopted, like standard of allowing drug withdrawal and not allow- ing drug withdrawal, drug withdrawal processes and disposal of withdrew drugs. The effects of the system were evaluated by com- paring the amount of withdrew drugs before （2008-Jun. 2009） and after （Jul. 2009-2013） the implementation of the system to make suggestions. RESULTS ＆ CONCLUSIONS： By establishing a method of detailed drug withdrawal management system combined with drug withdrawal amount and drug withdrawal department cost, the amount of withdrew outpatient and emergency prescription drugs decreased from 7.88 million yuan in 2008 to 90 000 yuan in 2013. Besides the establishment of reasonable drug withdrawal systems and the implementation of related management methods, it is suggested China Food and Drug Administration and hospitals should establish a unified package insert information classification database so the pharmacists can review prescriptions timely based on the information to reduce the phenomenon of drug withdrawal effectively.
