机械通气患者床头抬高与VAP发病率相关性调查分析

摘　　要：目的通过调查广州市3间三甲医院ICU实施机械通气患者床头抬高的现状,结合护士对床头抬高在预防呼吸机相关性肺炎(VAP)作用的认知,分析其与VAP发病率的相关性,为护理管理者提供管理参考依据。方法连续30天对广州市3间三甲医院综合ICU 88例机械通气患者床头抬高现状进行调查记录,结合自行设计调查问卷,对该3间医院ICU的133名护士进行床头抬高对预防VAP的认知度调查,并记录当月3间医院患者的VAP发病率。结果 B院床头抬高合格率为98.6%,显著高于A院的43.2%和C院的61.5%;B院护士对抬高床头预防VAP的认知度为96.2%,显著高于A院的10.2%和C院的34.4%;B院患者当月的VAP发病率为13.2%,显示低于A院的53.3%和C院的45.0%,差异均具有统计学意义(P<0.05)。结论 VAP的发病率与护士对机械通气患者床头抬高的依从性、认知度及护理管理者重视程度等因素有关。Objective By investigating the situation of bed-head elevation of ICU patients implemented mechanical ventilation in top three hospitals in Guangzhou, and combining with nurses＇ cognition about the role of bed-head elevation in preventing ventilator-associated pneumonia（VAP）, we analyze the relevance of the two factors with VAP morbidity, in order to provide reference for nursing management.Methods Situations of bed-head elevation of 88 ICU patients implemented mechanical ventilation in top three hospitals in Guangzhou were investigated for 30 consecutive days, the awareness survey of 133 nurses about bed-head elevation in preventing VAP were investigated by self-designed questionnaire, and the VAP morbidity in current month of three hospitals were recorded. Results The qualified rate of bed-head elevation in hospital B was 98.6%, significantly higher than 43.2% in hospital A and 61.5% in hospital C; the cognition rate of nurses about the role of bed-head elevation in preventing VAP of hospital B was 96.2%, significantly higher than 10.2% of hospital A and34.4% of hospital C; the VAP morbidity in current month of hospital B was 13.2%, obviously lower than 53.3% of hospital A and 45.0% of hospital C, all the difference was statistically significant（P 0.05）. Conclusions The VAP morbidity is relevant to the nurses＇ compliance and cognition about bed-head elevation of patients implemented mechanical ventilation and the attention degree of nurse manager.
