Preventive effect of warfarin on ischemia stroke in middle-aged and elderly patients with atrial fibrillation: a Meta-analysis

摘  要:目的评价华法林对中老年心房颤动患者缺血性卒中的预防效果。方法通过计算机检索2008年1月1日至2014年9月30日间Ovid、EMbase、Web of Science、中国知网、维普和万方数据库,收集华法林预防中老年心房颤动患者缺血性卒中的随机对照试验。基于纳入和排除标准对文献进行筛选,并采用Cochrane风险偏倚评估工具对纳入研究进行质量评价。采用Stata12.0软件进行Meta分析。结果共纳入6项随机对照实验,其中华法林组15959例,阿司匹林对照组14528例。Meta分析结果表明与华法林组相比,对照组出现缺血性脑卒中的风险是其2.8倍(OR=2.80,95%CI:1.35~5.82)。结论当前证据表明华法林可以降低房颤中老年患者患缺血性卒中的风险。Objective To review the preventive effect of warfarin on ischemia stroke in middle-aged and elderly patients with atrial fibrillation (AF). Methods The Databases of Ovid, EMbase, Web of Science, China National Knowledge Infrastructure (CNKI), VIP Database and WanFang Database were retrieved with computer from Jan. l, 2008 to Sept. 30, 2014 for collecting the randomized controlled trials (RCT) about the risk of ischemie stroke in middle-aged and elderly patients with AF treated with warfarin and aspirin. All literature was screened based on inclusion and exclusion criteria, and the included RCT were reviewed by using Cochrane risk bias tool. A Meta analysis was conducted by applying Statal2.0 software. Result There were totally 6 RCT included involving 15959 cases in warfarin gronp and 14528 in aspirin control group. The results of Meta-analysis showed that the risk of ischemia stroke is higher (by 2.8 times) in aspirin control group than that in warfarin group (OR=2.80, 95%CI: 1.35-5.82). Conclusion The current evidence shows that warfarin can reduce the risk of ischemia stroke in middle-aged and elderly patients with AF.
