歪鼻合并鼻中隔偏曲同期整复治疗的体会

摘　　要：目的 探讨歪鼻合并鼻中隔偏曲同期整复手术的可行性.方法 设计鼻小柱基底部至两侧鼻翼缘中部的蝶形切口,掀起鼻下端皮瓣,显露鼻中隔、鼻外侧软骨和鼻骨.对于软骨性歪鼻,分开大翼软骨、鼻外侧软骨和鼻中隔软骨之间连接是矫正的关键;对于骨性歪鼻,处理好鼻骨是矫正的关键.结果 本组共43例患者,随访12～ 24个月,满意38例（88.37％）,基本满意5例（11.63％）.结论 歪鼻合并鼻中隔偏曲联合整形术,可同期解决歪鼻的美容问题和鼻中隔偏曲而引起的鼻功能障碍问题,值得临床推广应用.Objective To explore the feasibility of combined plastic operation on the nasal deviation and the nasal septum deviation. Methods The butterfly cut from the base of the nasal columella to middle part of bilteral nasal ala was designed. The flap on the inferior nose was lifted to expose the nasal septum, lateral cartilage of nose and nasal bone. For the cartilaginous nasal deviation, it was the key to separate nasal lateral cartilage and correct the connecting of nasal lateral cartilage and cartilage of nasal septum ; for the osseous nasal deviation, it was important to reform the nasa bone. Results After 12 to 24 months following up, 38 were satisfied （88.37%） and 5 eases were satisfied basally （11.63%）. Conclusion The combined plasty of the nasal deviation and the nasal septum deviation could solve simultaneously the cosmetic problem of nasal deviation and the nasal dysfunction caused by nasal septum deviation. It is worth spreading in clinical.
