摘 要:目的分析我院万古霉素血药浓度的监测结果,促进临床合理用药。方法采用HPLC法测定万古霉素的血药浓度,对我院2012—2013年间测定万古霉素血药浓度的住院患者资料进行回顾性分析。结果74例患者共监测血药浓度116次,其中23例次(19.8%)血药浓度〈10mg·L-1,44例次(37.9%)血药浓度在10-20mg·L-1,49例次(42.2%)血药浓度〉20mg·L-1;疗效为显效的22例(29.8%),进步的33例(44.6%),无效的19例(25.6%)。结论开展万古霉素血药浓度监测十分必要,临床医师应根据血药浓度及时调整用药方案,实行个体化给药,避免不良反应。AIM To promote the clinical rational use of vancomycin by analyzing the therapeutic drug monitoring results in our hospital. METHODS HPLC was used to determine the plasma concentration of vancomycin. The clinical information of hospitalized patients received vancomycin and therapeutic drug monitoring from 2012 to 2013 was analyzed retrospectively. RESULTS Totally 116 cases of vancomycin plasma concentrations from 74 patients were monitored. There were 23 cases(19.8%) of plasma concentrations belowed 10 mg· L- 1. The plasma concentrations of 44 cases (37.9%) were between 10 and 20 mg· L- 1, and 49 cases (42.2%) were above 20 mg·L- 1. There were 22 patients (29.8%) of effective use,33 patients (44.6%) of little effect and 19 patients (25.6%) of inefective use in total 74 patients. CONCLUSION It is very important to carry out therapeutic drug monitoring of vancomycin. The doctor needs to modulate the dosage regimen and implement individualized adminisrtation according to the plasma concentration of van- comycin, so as to avoid the adverse drug reactions.
