摘　　要：目的探讨PCSK9 rs11206510基因多态性与脂代谢及发生冠心病(CHD)风险的相关研究。方法选择在天津市胸科医院住院患者778例,分为CHD组502例和对照组276例。入院后检验三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、载脂蛋白(Apo)A1、Apo B并检测入选人群的基因型TT、TC、CC。结果CHD组TC基因型组LDL-C水平较TT基因型组高(P<0.05)。CHD组和对照组均以TT基因型为主,2组基因型频率分布和等位基因频率分布差异均无统计学意义(P>0.05)。被检总人群等位基因频率分别为T 96.1%(1 495/1 556),C 3.9%(61/1 556);基因型频率分别为TT 92.2%(717/778),TC 7.8%(61/778)。多因素Logistic回归分析Apo A1、HDL-C是CHD的保护因素。结论 CHD组TC基因型与血清LDL-C的升高有相关性,而等位基因C与CHD的发病无明显相关性。Objective To investigate the relationship between PCSK9 gene polymorphism with lipid metabolism andCHD risk among population within Tianjin, China. Methods Patients from Tianjin Chest Hospital were selected（n=778）and divided into coronary heart disease（CHD） group（n=502） and control group（n=276）. Immediately after admission, tri-glyceride（TG）, high density lipoprotein（HDL-C）, low density lipoprotein（LDL-C）, apolipoprotein A（ApoA1） and apolipo-protein B（ApoB） were tested and genotype TT,TC and CC were determined. Results In CHD group, LDL-C level is higherin TC genotype individuals than that in TT genotype individuals with statistical significance（P〈0.05）. CHD group and thecontrol group were dominated by TT genotype with no statistical difference between their allele frequency distribution norgenotype frequency distribution（P〉0.05）. The total population allele frequencies were T 96.1%（1495/1556）, C 3.9%（61/1556）; genotype frequencies were TT 92.2%（717/778）, TC 7.8%（61/778）. Multivariate Logistic regression analysis revealedthat Apo A1 and HDL-C were inversely associated with coronary heart disease, therefore are protective factors for coronaryheart disease. Conclusion The study found that TC genotype is correlated with serum LDL-C increase in group CHD, andthe incidence of allele C showed no obvious connection with CHD.
