糖尿病病人并发症数量及出现时间与自我管理行为的关系

摘　　要：[目的]探讨糖尿病病人并发症数量、出现时间与自我管理行为的关系,为降低急慢性并发症的发生率,提高病人的自我管理水平提供依据。[方法]采用横断面调查的方法,对内分泌科糖尿病并发症病人217例,应用一般情况调查表及糖尿病自我管理测评量表进行调查,并按糖尿病并发症数量及出现时间,分析其各维度自我管理水平。[结果]糖尿病病人并发2个及以上并发症者比并发1个并发症者自我管理行为在饮食维度、运动维度、遵医嘱用药得分低,差异有统计学意义（P〈0.05）;并发症出现时间10年及以上的病人比并发症出现时间5年~10年、5年以下的病人自我管理行为中运动维度得分低,差异有统计学意义（P〈0.05）。[结论]糖尿病并发症数量少的病人饮食、运动、遵医嘱用药自我管理行为水平高;糖尿病病人并发症出现时间长,自我管理行为中运动维度水平低。Objective：To probe into the numbers,the time of occurrence of complications and self - management behaviors of diabetic patients,so as to provide evidences for reducing the incidence of acute and chronic compli- cations and to improve patients＇ self - management level. Methods.A cross - sectional survey method was used to investigate 217 diabetes patients with complications in department of endocrinology by using the general questionnaire and the Summary of Diabetes Self- Care Activities Measure,according to the number of diabetic complications and time of occurrence to analyze the various dimensions of self - management level. Results：The score of self - management behaviors of diabetic patients who complicated with two or more complications was lower than that of patients who complicated with one complication in diet dimension, exercises dimension and medication compliance score. There were statisticall significant differences among them（P〈0.05）. In terms of exercises scores in self - management behaviors,the score of patients with the time of occurrence of complica- tions 10 years or more is lower than those of complications 5 - 10 years and 1 - 5 years time. There was statisti- cal significant difference between them with（P〈0.05）. Conclusion： Patients with less diabetic complications the level of self - management behaviors including diet, exercise and medication compliance is high. Diabetes patients with long time of complications of exercise dimension level in self- management behaviors is low.
